Regarding OPVEE (a high dosage naloxone product), completely unnecessarily high dosage. The gold standard is the IM Naloxone (injectable naloxone) the rock star of naloxone.
"They’re pointing out that the existing research was all conducted on “healthy” subjects, meaning non opioid dependent people. For them they worked great and had no side effects.
They are saying that there needs to be more research into real world situations before it can be determined if these cause harm to people with tolerance.
The manufacturers of the products cite these studies of healthy subjects and used medical modeling to help fast-track these products through the FDA with no clinical trials, citing that there’s no ethical was to construct such a trial. I recall reading an interview with Nab who acknowledged that it would be tricky, but not impossible. I imagine that utilizing these products in an ED with data collected by as close to impartial people as possible might be one way to go about it since the means to mitigate the potential side effects would be available.
As it is, the few post release studies that the manufacturers undertook collected data from EMS and law enforcement who all noted no significant side effects, but did not include any information about the individuals who received the meds, so it’s not much different than the anecdotal reports that were also used to justify and approve these products, which were pretty much “it took 8 Narcans to bring him back.”
