We now have a new center open in Lauderdale Lakes (3736 W Oakland Park Blvd). Community Care Plan, the health plan with a heart, was established in 2000 and is owned by Broward Health and Memorial Healthcare System. CCP is based in Sunrise, Florida, was the first and remains the longest-standing Provider Service Network (PSN) in the state. Community isn’t just part of our name; it truly represents

who we are. Our mission is to build healthier communities and we do so by being the driving force to ensure that every community has access to high-quality affordable health care. Our health plans cover a wide range of medical services and offer superior quality physicians and other benefits that help members get and stay healthy. We serve more than 100,000 members in Florida Healthy Kids, Medicaid; commercial, self-insured employee health plans; and sponsored programs, and partner with more than 130 community organizations. Community Care Plan is accredited by the National Committee for Quality Assurance (NCQA) and Accreditation Association for Ambulatory Health Care (AAAHC) as a Health Plan.

Back-to-School Success Starts with Community. 🍎

Thank you to everyone who supported our Pencils & Possibilities event on July 11. Together, we provided 264 backpacks, 45 uniforms, and offered 16 community resources (Early Learning Coalition, FAU Broward Up, Dade County Federal Credit Union, and more), with 109 families served! 💙📚🎒✏️

📚 Join us for a fun and supportive back-to-school event! Free backpacks will be available while supplies last, along with school uniform items, and other essential back-to-school supplies.

🗣️ Also, get the opportunity to connect with local community organizations on-site for valuable resources and services for families.
