At 4pm, one team hands over to another. The day team shares the important details, updates and knowledge that help the night team continue providing safe, consistent and person-centred support.

Then, in the morning, the process happens all over again as the night team hands back to the day team.

It’s so much more than a change of shift. It’s about communication, trust, professionalism and teamwork. It’s about knowing the people we support, understanding their needs and making sure nothing is missed.

In supported living, our frontline workforce is there around the clock — supporting routines, promoting independence, offering reassurance and making a difference to everyday lives.

👏 This is frontline social care at its best. Dedicated professionals working together, sharing responsibility and putting the people they support at the heart of everything they do.

At Freedom Support, every handover matters, every detail counts, and every member of the team plays their part. 💙
