You know they’re ready to go home.

Clinically cleared. But discharge stalls.

Not because they still need a hospital bed - but because what’s needed at home isn’t ready yet.

Support. Transport. Equipment. Home modifications.

Someone to coordinate it all and make sure nothing falls through the cracks.

That’s often the pressure point.

At Freewave Aged Care, we help make discharge happen.

We move quickly to put the right supports in place so older people can leave hospital safely and settle back into the place they want to be - home.

Whether that means immediate in-home support, assisted transport, short-term restorative care, assistive technology, home modifications or ongoing care, we work quickly to coordinate care and remove delays.

Because discharge is not just about getting someone home.

It’s about helping them stay home safely.

🔹️ Rapid response and mobilisation

🔹️ Door-to-door assisted transport

🔹️ Immediate in-home support

🔹️ Assistive technology and home modifications

🔹️ Restorative and ongoing support pathways

🔹️ Clinical oversight and coordination where needed

🔹️ Continuity of care, without the gaps

We answer the phone.

We respond.

We take ownership.

And we follow things through.

Because when someone is ready to go home, timing matters.

Back to their own home. Their own bed. Their own routine.

That matters.

We’re seeing this every day.

Co-contributions are pushing people to make choices they shouldn’t have to - between essentials and the care they need.

You don’t have to go without. Hardship provisions exist, and there’s no shame in using them.

If you need support, apply for a fee reduction or waiver - or reach out. We’ll help you get it sorted.

If fees from your Support at Home package are becoming difficult to manage, you can apply for financial hardship assistance through Services Australia which will determine your eligibility. Make sure you take the time to understand both the requirements and the eligibility criteria before applying.

There are options to support your situation which may include having your fees waived, reduced, or deferred for a period. You have the right to ask for support without judgment.

OPAN has created a fact sheet to help you understand the options available if you’re finding it difficult to pay aged care fees.

To view the fact sheet, visit our dedicated New Aged Care Act page: https://opan.org.au/new-aged-care-act/
