A day of learning, connection and understanding! 💚

Our Participant Services team spent the day taking part in DISC training, exploring different communication styles, personalities and how they can work better together.

Understanding ourselves and each other helps us build stronger teams, improve communication and ultimately provide better support to our participants.

Always learning, always growing!

We invest deeply in training at all levels, beyond mandatory and funded expectations because we value skills and knowledge in our services.

Making the most of our very own gym! 💪💚

Our support worker Cody has been putting our gym space to great use during his shifts, supporting one of our participants to stay active, build confidence and work towards their fitness goals.

It’s a great example of how everyday supports can be tailored to each participant’s interests, goals and wellbeing!

What makes Community Care Foundation different from every other disability support provider?

A great morning at Coffee with Jess! ☕

Coffee with Jess is an opportunity for our CEO to connect with everyone across our team, from frontline support workers to managers, over a coffee and a good conversation.

🏡 From Hospital to Home, With the Right Support in Place.

Leaving hospital after a significant illness or injury can feel overwhelming, especially when complex support needs are involved.

At Community Care Foundation (CCF), we work alongside participants, families, hospital discharge teams and healthcare professionals to help make the transition from hospital to home as smooth, safe and supported as possible.

Whether it's finding suitable accommodation, arranging Supported Independent Living (SIL), coordinating complex supports or planning for ongoing care, we're here to help every step of the way.

💚And we've seen firsthand what great collaboration can achieve!

Through the combined efforts of Glenn, Nurse Navigator, and Elisa, our Participant Services Leader, we've had some wonderful success stories helping participants transition from hospital into the community with the right supports in place.

Because a successful transition is about more than leaving hospital. It's about feeling safe, supported and confident in what comes next.
