Sometimes, getting out of the house is a huge win. 💚

Izzy has been getting out and about in the community, building confidence, and working towards goals that are meaningful to him.

Every goal looks different, and every step forward deserves to be celebrated. We love seeing Izzy out, smiling and making the most of his day!

At Community Care Foundation, we support participants to work towards their individual goals at their own pace.

Today is World Duchenne Awareness Day 🎈

Duchenne Muscular Dystrophy is part of Max’s life, but it is only one part of who he is.

In this video, Max helps us learn a little more about Duchenne.

At Community Care Foundation, we believe in listening to lived experience, learning from the people we support, and creating communities where everyone has the opportunity to be included, heard and understood.

Thank you, Max ( Shorter Brothers ), for sharing your voice and helping us raise awareness today.

Community Care Foundation is proud to support our local community by sponsoring a player for the Hervey Bay Dingoes 🏀

We’re proud to help create opportunities for people to participate, connect and be part of something they enjoy.

Wishing the Dingoes and Emerald a great season ahead!

🐾 Welcome to the pack, Community Care Foundation! 🏀

The Hervey Bay Dingoes are proud to welcome as a Player Sponsor for Emerald.

Community Care Foundation is a registered charity and not-for-profit organisation providing disability support services across the Fraser Coast. Their work focuses on inclusion, community participation, independence and helping people with disability achieve their goals.

As a registered provider for both NDIS and NIISQ participants, Community Care Foundation is deeply committed to creating inclusive communities where people with disability can participate fully, build connection and be supported in the things that matter to them.

That is exactly why this partnership is such a beautiful fit.

By coming on board as Emerald’s Player Sponsor, Community Care Foundation is helping support her wheelchair basketball journey and reducing some of the practical costs that come with training, competing and staying involved in the sport she loves.

This partnership helps create more than sporting opportunity. It helps build confidence, social connection, community participation and a stronger sense of belonging.

Thank you, Community Care Foundation, for backing Emerald and helping us put more players on court.

Welcome to the pack. 🧡💙

Could the right home, the right support and the right people make all the difference? 🏡💚

Community Care Foundation currently has Supported Independent Living (SIL) opportunities available in Hervey Bay and Maryborough.

For us, SIL is about more than having somewhere to live. It’s about creating a home where people can feel safe and supported while building independence, confidence and working towards the life they choose.

Our team has experience supporting people with a range of needs, including more complex and changing support needs, people living with dementia and people transitioning back into the community following lengthy hospital admissions.

We take the time to understand the person, their goals and what matters to them, working alongside families, Support Coordinators, Allied Health professionals and other supports to find the right fit.

📍 Current opportunities in Hervey Bay & Maryborough

Looking for a new home or supporting someone who may be ready for a change? Get in touch with our team to discuss our current vacancies and find out whether one of our homes could be the right fit.

Fraser Coast, this is a conversation you won't want to miss. 💙

If you're a parent, family member, carer, disability support professional, or anyone supporting a person with disability, join us for a powerful and inspiring evening with bestselling Australian author Fleur McDonald.

Many families in our community understand the unique challenges, sacrifices, joys, and resilience that come with raising and supporting a loved one with disability. Fleur's story speaks directly to that journey.

While many know Fleur as one of Australia's most successful rural fiction authors, her greatest achievements have happened far beyond the pages of her books. While working on the land, she raised two children with disabilities, cared for her terminally ill mother-in-law, overcame significant personal adversity, and pursued her dream of becoming a writer against all odds.

Her story is one of determination, resilience, advocacy, and finding strength during life's toughest seasons. It's a story that many disability families will relate to, and one that will leave you feeling inspired, understood, and empowered.

✨ An intimate evening of storytelling, conversation, connection, and hope.

🎟️ Limited tickets available: https://www.trybooking.com/au/event/1629359

Whether you're walking your own caregiving journey, supporting a family member, or working in the disability sector, we would love to see you there.

Come and enjoy a two course meal, while Fleur speaks about her life as a farmer, best selling author, domestic violence survivor and raising two child
