The Massachusetts Department of Public Health Community Health Equity Survey (CHES) is now live!

This is your opportunity to make sure your community’s priorities show up in the data that state government and community organizations. We invite you to complete the survey if you have not already done so and to share it broadly with your programs, vendors, contractors, grantees, community partners, and the residents you serve.

The Community Health Equity Survey gathers input directly from Massachusetts residents about the social, economic, and environmental factors that influence their health and well-being. The information collected will help the Massachusetts Department of Public Health:
-Better understand community strengths, priorities, and barriers to health.
-Identify the root causes of health inequities.
-Inform funding and resource allocation.
-Strengthen policies, programs, and services to better meet community needs.
