🌺 A lovely afternoon visiting our partners at Jubilee Housing - one of the organizations in our new collaborative grant for Ward 1 - the Connected Care & Aging Network (C-CAN).
The grant supports our mission by providing options for older adults to age in place by connecting older-adult affordable housing with community-based programming.
Jubilee Housing offers individualized service coordination, health and wellness supports, food access, transportation, digital literacy, and community programming through strong local partnerships to help older adults age in place and reduce social isolation. ☕
🎨 Story Tapestries is also part of the collaboration and will deliver four culturally responsive, art-based workshops at Jubilee Housing to support memory, mental health, and social connection among older adults in permanent supportive housing.
