As part of our planning for ACU’s Health Center Rescue Network, we are conducting a survey to learn more about the disaster preparedness and response resources currently available to community health centers (CHCs), recent experiences with disaster response, and current priorities and needs.

We invite anyone working at a CHC, PCA, or HCCN to complete this survey, especially those whose work involves compliance with Emergency Preparedness Requirements for CHCs or coordination and oversight of disaster preparedness and response efforts.
