Somali community health survey
Assalaamu Alaykum brothers and sisters,
My name is Ahmed Elmi, I am the chairman of the Somali American Community Association (sacausa.org) in Maryland. We are a volunteer-based organization with the purpose of assisting and empowering the Somali community to the best of our abilities. Over the years, we have provided advocacy and supportive services, including health, education and immigration.

This short community health survey will help us understand the health and health care needs of the Somali community. The results may help us provide the Somali communities: 1) information on health and well-being, and 2) health-related resources in various states. We are not getting money from any sources to do this survey.

Before you take this survey, we need to tell you the following:

You must be at least 18 years old to take this survey.

We are not collecting your personal information.

You are volunteering to take this survey.

We don’t see risks or discomforts for taking this survey.

We are not providing you money or gifts for taking this survey.
