Do you have a child with a disability between the ages of 3 and 21? We invite you to complete a survey about your child’s participation in physical activities and sports, including dancing, walking, playing, and other forms of movement.
Your feedback will help us understand the challenges and supports that affect participation and develop educational resources to increase awareness and encourage involvement.
This survey is part of an MUSC Occupational Therapy Doctoral Capstone project conducted in collaboration with the City of North Charleston Recreation Department’s Therapeutic Recreation Division.
The survey takes approximately 10–15 minutes. Please complete a separate response for each child with a disability ages 3–21 by October 21, 2026.
