Parish Census & Survey

Dear Parishioners of St. Joachim Church,

On behalf of the parish leadership, I am asking each household to help us update our parish records. Accurate information allows us to serve you better in areas such as sacramental preparation, pastoral care, communication, ministry planning, and diocesan reporting.

Please complete one form per household, even if you believe your information is already up to date. The deadline to submit the form is September 7.

All information will be kept strictly confidential and used only for parish purposes.
