We coordinate personalized services for people with physical, intellectual/developmental disabilities, autism and older adults.

We use person-centered planning to empower our participants to live the life they envision for themselves. SCU coordinates services for individuals with physical disabilities, intellectual/developmental disabilities, autism and older adults helping them live independently in their homes and communities. We work with various state programs and waivers using person-centered planning focusing on individual needs and preferences. We empower our participants to make choices about their own care and connect them to services that enhance their independence. These services include home care, nursing, meals, transportation, community participation, home modifications and more. Our coordinators assist our participants, along with their family or team, to live the life they envision for themselves
