Community Clinic Association of Los Angeles County Representing LA County's non-profit community clinics and health centers that deliver cost-effective, high quality health care. www.ccalac.org

The Community Clinic Association of Los Angeles County (CCALAC) and our members share a common mission of supporting and expanding access to quality comprehensive health care for every individual. Specifically, CCALAC’s mission is to, “promote free and community clinics as providers and advocates for expanding access to quality comprehensive health care for medically underserved people in Los Angeles County.”

Our vision is to, “advance the health and human services of communities throughout Los Angeles County, creating a comprehensive health care system for underserved populations to help reduce health disparities in the county.”

HISTORY OF CCALAC

After the civil unrest in Los Angeles County in the 1990s, the LA-based National Health Foundation was seeking ways to increase access to primary medical care in poor neighborhoods. The Community Clinic Association of Los Angeles (CCALAC) was founded in 1994 as part of that project. In 1996, CCALAC obtained its IRS (501)(c)(3) tax exemption status. Since then, our organization has grown to be the largest regional association of community and free clinics in California.

How do you turn population health data into daily action? 🏥 As value-based care expectations expand, community health centers must find ways to operationalize data across clinical, quality, and operational workflows.

This session features a multi-state FQHC sharing practical insights from implementing a population health platform across 16 clinical locations in California, Arizona, and Nevada. Discover how their cross-functional teams leveraged a shared data infrastructure to close gaps in care, improve documentation accuracy, and strengthen referral tracking. 📊

Attendees will review measurable outcomes—including improvements in breast, to***co, HIV, and cervical cancer screenings—and gain scalable strategies for enterprise-wide workflow alignment.

Key takeaways include:
✅ Embed practical workflows into clinical, referral, and front-office operations to improve care coordination.
✅ Leverage integrated EHR tools and analytics to monitor referral performance and identify documentation discrepancies.
✅ Apply scalable strategies for cross-functional technology adoption across multiple sites and departments.
