We’re mislabeling a lot of veterans — and it’s costing them more than we want to admit.

Not everything is PTSD.

And forcing it into that box isn’t helping.

PTSD is rooted in fear. It’s what happens when your brain won’t stand down after a threat is over.
Hypervigilance. Nightmares. Avoidance.
Your system acting like the danger is still in the room.

But some people aren’t reliving fear.

They’re reliving decisions.
Moments.
Things they did… or didn’t do.

That’s moral injury.

And it doesn’t sound like: “I’m not safe.”
It sounds like: “I don’t know what this says about me.”

Different wound. Different problem.

You can’t out-breathe that.
You can’t checklist your way through it.
And you definitely can’t fix it by pretending it’s the same thing as PTSD.

I spent 20 years in the Air Force. I wasn’t in the roles people usually point to when they talk about trauma, but I saw what people carried.

I watched people get quieter over time — not because they were afraid, but because something in them didn’t sit right anymore… and there was nowhere to put it.

No category. No language. Just weight.

We had systems for everything.

Just not that.

I’ve sat with veterans who don’t meet PTSD criteria. Nothing obvious on paper.

But they can’t fully show up at home.
They brush off respect like it doesn’t belong to them.
They carry an ongoing question about who they are now — and whether they’re okay with the answer.

That’s moral injury.

And we keep missing it.

Because we keep trying to treat meaning like it’s fear — and then wondering why it doesn’t fully land.

So when it doesn’t work, people don’t blame the model.

They blame themselves.

“Guess I’m just the exception.”

They’re not.

We’re just asking the wrong questions.

Moral injury requires a different conversation — one about values, responsibility, and the stories people carry about who they are.

It’s not quick.
It’s not clean.
But it’s real.
