Rocky Mountain Health Care Services grows with our community’s unique and ever-changing needs. Currently we have one outstanding programs: Rocky Mountain PACE. Rocky Mountain PACE (Program of All-inclusive Care for the Elderly)

The PACE model of care—Program of All-inclusive Care for the Elderly—is considered the gold standard of care for seniors nationwide. PACE is enriching the lives of senior through coordinated care. Seniors who work with our team receive medical and support services to empower them to live as independently as possible in their own home or living community. Our goal is to help seniors maintain the highest level of independence as possible. Rocky Mountain PACE is the only national PACE program in the region, offering a holistic suite of services to meet the health care needs of seniors in our community. Seniors and their families do well if they have a role in their own health and we see ourselves as partners in their wellbeing. From their physical, social and emotional health, to the simple things like transportation and supportive services that help them access their care, we partner with seniors in keeping them as independent as possible. Our seniors experience their health care delivery with a coordinated, holistic and integrated approach. Our team discusses health care treatment options from many vantage points which gives our participants a strong likelihood for healthy outcomes. Rocky Mountain PACE is gaining national attention for our exceptional behavioral health integration, which is not a required discipline to operate a PACE program. We also are actively sought after to provide consulting services to other PACE and health care programs throughout the nation. Our notoriety stems from our exceptional financial stewardship, ability to cut administrative costs and waste utilizing Lean Six Sigma methodology, and our tremendous success in executing a rapidly growing PACE program.

October is Breast Cancer Awareness Month.
As we age, prioritizing preventive healthcare becomes increasingly important. Older adults are encouraged to stay informed about recommended screenings, discuss personal risk factors with their healthcare providers, and maintain regular wellness visits. Early detection can lead to better treatment outcomes and improved quality of life.

Breast cancer affects individuals, families, and communities, making awareness and education essential at every stage of life. Let us continue to promote access to healthcare, support caregivers, and empower older adults to take an active role in their health journey.

Honoring Indigenous Peoples' Day
Today, we recognize and honor the Indigenous peoples whose cultures, traditions, knowledge, and contributions have shaped our communities and our nation for generations.

Indigenous Peoples' Day is an opportunity to reflect on the rich history, resilience, and enduring presence of Native peoples, while acknowledging their important role in preserving cultural heritage, stewarding the land, and strengthening communities.

As we commemorate this day, let us take time to learn, listen, and celebrate the diverse Indigenous nations and peoples whose stories continue to enrich our shared future.

World Mental Health Day

Mental health is an essential part of overall well-being at every stage of life. At Rocky Mountain PACE, our social workers and counselors play a vital role in supporting participants' emotional health and well-being. Through compassionate listening, guidance, advocacy, and counseling services, they help participants navigate life's challenges while fostering connection, resilience, and a sense of belonging.

This World Mental Health Day, we extend our gratitude to the dedicated professionals who care for the whole person, not just their physical health, but their emotional well-being as well. Together, we can create a culture where mental health is openly discussed, support is readily available, and every individual feels seen, supported, and empowered to thrive.

Happy National Scrubs Day to the amazing RMPACE team!
Scrubs may not come with capes, but we're pretty sure they're the uniform of everyday superheroes. Whether you're caring for participants, supporting families, solving problems, or keeping things running smoothly behind the scenes, you make a difference every single day.

Thank you for all you do to keep RMPACE healthy, happy, and moving forward. Today we celebrate the people who prove that compassion never goes out of style.

This World Podiatry Day, we're recognizing the important role foot care plays in helping Rocky Mountain PACE participants maintain their independence and quality of life.

Our dedicated nursing team works closely with participants to monitor and address their podiatry needs, helping prevent complications before they become bigger concerns. From routine foot assessments to coordinating specialized podiatry care, our nurses play a vital role in keeping participants comfortable, healthy, and moving safely.

By preventing complications and improving comfort and mobility, dedicated foot care helps our participants stay active, engaged, and connected to the people and activities they enjoy most.
We're grateful for our nurses, podiatry professionals, and care teams who work together to make those everyday moments possible, one step at a time.

Every step matters at Rocky Mountain PACE.

National Inner Beauty Day
Today, we celebrate the qualities that make people truly remarkable: kindness, compassion, resilience, wisdom, and generosity.

Older adults remind us every day that beauty is not defined by appearance, but by the lives we touch, the experiences we share, and the strength we show through life's challenges and triumphs. Their stories, guidance, and unwavering spirit enrich our families, communities, and generations to come.

On National Inner Beauty Day, let's take a moment to recognize and appreciate the older adults who inspire us with their grace, character, and heart. Their inner beauty shines brightly and leaves a lasting impact on everyone around them.
