This week, two celebrations come together around one shared purpose: connecting people with the care and support they need.

While we continue to celebrate Illinois Free & Charitable Clinics Month, we are also proud to recognize National Community Health Worker Awareness Week, August 24-28, as celebrated by the National Association of Community Health Workers (NACHW).

Community Health Workers (CHWs) are trusted neighbors, connectors, advocates, and guides who help people navigate the healthcare system, overcome barriers, and access important services and resources.

This same spirit of connection is also at the heart of free and charitable clinics. Together, CHWs, clinics, volunteers, healthcare professionals, and community partners help make care feel a little more accessible for the people who need it.

This week, we celebrate CHWs everywhere and thank them for the relationships they build and the difference they make in their communities. 🎉💙⭐🎊

August 24-28 marks the fourth annual National Community Health Worker Awareness Week! We’re proud to recognize our dedicated CHDC Community Health Workers (CHWs), Gladys and Bryanna (pictured left to right). Help us thank them for the incredible work they do each day by liking, commenting on, and sharing this post. 🎉💚🗨️

As part of the Montgomery County Street Medicine Program, our CHWs provide essential healthcare services to individuals experiencing homelessness in Pottstown and beyond. Founded in January 2021 by Tower Health Medical Group and Access Services, Inc., the program continues to make a meaningful impact through trusted partnerships and dedicated community care.
