Interested applicants for the BS in Public Health program for the Academic Year 2026–2027 who were not able to attend the previous screening may still come until the second week of June, from Monday to Thursday only.

Applicants are required to bring two (2) sets of long envelopes containing the following documents:

📄 Original and Photocopy of:
• PSA Birth Certificate
• Form 137
• CSU-CAT Result
• Student Profile
• Good Moral Certificate
• Two (2) pieces of colored 2×2 ID pictures with white background
