✅ You may also secure a copy from your PICPA Chapter Office or print the form sent via email by the PICPA National Office
✅ Fill out the form with your chosen candidates, or allow your designated representative to do it for you
✅ Don’t forget to originally sign the form and attach a photocopy of your government-issued ID or PICPA ID
