We aim achieve this by educating, innovating and collaborating so that people are empowered to care with confidence and compassion at end of life. Our Public Health team work at the heart of local communities. We provide resources and courses to help with end of life planning and coping with bereavement. We also support carers, volunteers and whole communities to care together, with confidence. Wh

We’re proud to launch The End of Life Partnership’s latest Impact Report 💙

The report shares the difference we’ve made together over the past year through education, collaboration, innovation and leadership in end of life care.

In 2025, 1,845 professionals from 244 different organisations accessed our training and education programmes, helping build confidence and compassionate care across health and social care settings.

Inside the report, we highlight:
🔹Our leadership programmes supporting future leaders in palliative care and hospice services
🔹Work leading the Population Based Needs Assessment for Cheshire & Merseyside ICB leading to better decision making
🔹Co-production projects helping people influence health and social care decisions
🔹A data dashboard helping improve the quality of palliative and end of life care in Warrington Place

We’re pleased to invite family members to a supportive and informative Understanding Dementia – Family Talk Session 💬

📅 Wednesday 27th May
🕒 Morning or Afternoon (your preference)
⏱️ Duration: Approx. 1½ hours

This session will focus on understanding dementia and how it can affect your loved one, including:
• Changes in memory, behaviour, and communication
• Practical ways to support and respond with confidence
• Guidance on providing reassurance and reducing anxiety

There will also be time to ask questions and share experiences in a relaxed and supportive environment.

All family members are welcome, and we would be especially pleased if residents’ relatives are able to attend.

💜 Introducing ICAN Dementia Training 💜

Interaction. Communication. Namaste.

This Dementia Action Week, we’re proud to spotlight our new ICAN Dementia Training Programme developed to support compassionate, person-centred dementia care across health and social care settings.

Built on respected dementia care approaches, ICAN brings together interaction, communication and Namaste Care practices to support meaningful, relationship-centred care for people living with dementia.

The programme draws upon:
☑️Teepa Snow’s Positive Approach to Care™
☑️Joyce Simard’s Namaste Care
☑️Professor Ian James’ 8 Needs Model

Over five 2-hour sessions, participants will explore how to:
✔️Strengthen person-centred dementia care approaches
✔️Recognise and overcome communication barriers
✔️Apply verbal, non-verbal and therapeutic communication techniques
✔️Develop meaningful care plans that support emotional wellbeing

Delivered by dementia specialists with backgrounds in Admiral Nursing and mental health occupational therapy, this five-week online programme has been created to support professionals across care homes, community services, hospices and healthcare organisations.

For people living with advanced dementia, connection, comfort and meaningful moments matter deeply.

As part of Dementia Action Week, we’re highlighting our Namaste Care Train the Trainer programme, designed to help care organisations confidently introduce compassionate, person-centred Namaste Care into everyday practice.

Suitable for dementia care trainers and health and social care staff supporting people living with advanced dementia in any setting.

The course includes:
✨ A full training day
✨ A follow-up online mentoring session
✨ A free namaste starter basket, including sensory items
✨ A comprehensive resource pack

Delivered by experienced dementia and end of life care specialists, this programme supports teams to create calmer, more meaningful experiences for the people they care for.

Free Community Support Sessions in Wilmslow

Supporting someone through illness, frailty, or later life can bring many questions, emotions and changes, and it’s not always easy to know where to turn for support.

At EOLP, we are committed to creating spaces where people can learn, reflect and feel supported within their community. This June and July, we are offering three free community sessions at The Guild for Lifelong Learning in Wilmslow.

📅 Thursday 11 June - “Confidence to Care at End of Life” session
Build confidence and understanding by exploring what may happen towards the end of life, helping to reduce fear of the unknown and supporting people to feel more prepared and present.

📅 Thursday 25 June - “Grieving Before Loss” session
Feel heard and supported by recognising the emotions that can arise long before a death. This session offers a safe and compassionate space to reflect on changing relationships, uncertainty and anticipatory grief.

📅 Thursday 2 July - "Grieving When Caring Ends" session
When a caring role ends, it can leave a huge gap in daily life and identity. Learn how to navigate life after caring, cope with change and loss, and find a way forward.

All sessions offer an opportunity to meet others who may be experiencing similar challenges, helping you to feel less alone.

It’s a chance to say thank you to the incredible nurses who care for people and families every day.

At The End of Life Partnership, nursing is a big part of our story.

Our facilitators and leadership team started their careers in nursing, and that experience continues to shape everything we do.

We had a great time at the Wilmslow Health & Wellbeing Fair on Saturday – such a warm and welcoming event, brilliantly organised by the Chelford, Handforth, Alderley Edge and Wilmslow Care Community.

Suzanne, our Public Health and Dementia Worker, was there demonstrating how we support local neighbourhood teams and the communities they serve.

Suzanne is working closely with the CHAW Care Community to help bring the right support for the community. She spoke with attendees about our carer education sessions, how we can help people plan ahead for their future care, and the wider support we offer, including the bereavement and dementia networks, and Namaste Care.
