CHoiCe Trust CHoiCe Trust aims to support the rural communities in Mopani District, Limpopo Province, South Africa by addressing health and social concerns.

Through the 2 programmes of CHoiCe Trust - Outreach, and Training and Development - the staff of the organisation are positively impacting on the lives of children, patients, family members, caregivers and farm workers. The Outreach Programme provides the following services:
- Orphans and Vulnerable Children Project
- Palliative Care Project
- HIV Counselling and Testing services
- Home-Based Care

Mentoring support
- Farms Project

Through the Training and Development Programme, CHoiCe is able to provide both accredited and non-accredited health trainings to caregivers as well as community members. Some trainings include HIV, AIDS and STIs; Tuberculosis; Home Care; and Palliative Care.

HIRING: We are seeking a Social Worker: Palliative Care and Family Support to support the implementation of quality palliative care to all patients served by CHoiCe Trust. This includes, but is not limited to, the assessment of referred patients, the development of a care plan, working to reduce and control pain and other symptoms with patients and working in cooperation with different stakeholders to ensure that the patient’s quality of life has improved. The purpose of this position is to give direct support to households based on referrals and the development of care plans. The social worker is required to strengthen the quality of the services provided to address social needs, particularly as they impact on HIV vulnerability and treatment adherence.

Required Qualifications, Skills and Qualities:

The appointed individual is expected to have the following experience and qualifications:

- Social Work Degree or equivalent and relevant qualification

- Registered with the SACSSP and eligible to practice as a Social Worker in South Africa

- Knowledge of the health and development sector

- Minimum of 5 years’ experience in working in the palliative care/OVC field

- Experience with children/adolescents living with HIV or patients would be an advantage

- Ability to communicate in local languages will be an advantage

- Good written English skills

- Driver’s license and ability to drive

The Social Worker must be passionate and compassionate, with a commitment to the support of children.

He or she must work well with people, both external stakeholders and within a team, must be goal-oriented, and deadline driven. The social worker must be able to advocate on behalf of children, palliative care patients, and their families – with perseverance, assertiveness and tenacity.

They must have good observation skills and be able to innovate and plan around the best interests of patients, their families and of children, with a key focus on children, adolescents and caregivers living with HIV.

The individual will be required to provide a Police Clearance Certificate prior to employment.

Responsibilities include:

- Achievement of project targets

- Implementation of quality services according to set standards

- Support mental health and psychosocial support needs.

- Stakeholder engagements

- Monitoring and Reporting

- General Support

NOTE: We offer a market related salary. The position is based in Tzaneen with regular travel, mainly within Mopani District.
