Subject: Multi-Vehicle Accident on N1 Involving TMPD Vehicle
Date of Incident: 07 August 2026
Time of Incident: Approximately 21:40
Location: N1 and N4, Silverton

On Friday evening, 07 August, the Old East Safety Forum responded to a multi-vehicle accident on the N1 highway, where a TMPD vehicle was struck from behind while stationary. On arrival, the driver of the other vehicle involved and the TMPD members were treated on scene by Medi Edge Emergency Medical Services and Advanced Paramedic Assist. Our members assisted with scene safety while the occupants were treated, allowing the vehicles to be cleared from the road safely and without further incident.

On Friday evening, 07 August, the Old East Safety Forum responded alongside other agencies to a single-vehicle accident at the corner of Nicolson and Walter Lanham Street, Bailey's Muckleneuk, after reports of a car colliding with a wall. Fortunately, the driver walked away uninjured, and her vehicle was safely removed from the wall.

Subject: Multiple Shack Fires at Centurion Taxi Rank Informal Settlement

Date of Incident: 25 July 2026
Time of Incident: Approximately 23:50
Location: Corner of South Street & Hendrik Verwoerd Drive, Centurion

On Saturday evening, 25 July 2026, multiple shack fires broke out at the Centurion Taxi Rank Informal Settlement. The Old East Safety Forum responded alongside other agencies and organisations, assisting with crowd control, supporting the City of Tshwane Emergency Services Department on scene, and helping affected families in the aftermath of the fires.

At the time of this report, no fatalities had been recorded. One patient was transported by Gauteng EMS to a nearby hospital for minor injuries. The cause of the fire remains under investigation by SAPS.

SUCCESS REPORT

Subject: Cellphone Theft Operation - Suspects Apprehended
Date of Incident: 11 July 2026
Location: Loftus Versfeld Stadium and Surrounding Areas, Pretoria

On Saturday evening, 11 July 2026, members of the Old East Safety Forum participated in a coordinated anti-cellphone theft operation in and around Loftus Versfeld Stadium. The operation was conducted in partnership with the South African Police Service (SAPS), ProShield Security, Hatfield CID, Axon Security, Tactical Edge Security, and Fidelity-ADT.

Prior to the event, identified hotspot areas were assessed, and operational resources were deployed accordingly. The proactive deployment proved effective, with only one known cellphone snatching reported within the areas covered by the operation. Spotting teams successfully identified suspicious activity throughout the evening, contributing to the overall success of the operation.

During patrols on Park Street, SAPS members observed a white Audi A4 travelling at high speed under suspicious circumstances. With the assistance of private security partners, the vehicle was instructed to stop. The driver failed to comply and fled the scene at high speed, resulting in a pursuit. The vehicle subsequently lost control in Stanza Bopape Street, mounted the sidewalk, and sustained damage to its right front suspension before coming to a stop.

The occupants, three African males, were detained, and the vehicle was searched. Officers discovered that the vehicle had been fitted with false registration plates. A further search revealed one black iPhone 12 wrapped in aluminium foil, a common method used in an attempt to prevent the device from being tracked. The occupants were unable to provide a lawful explanation for possession of the cellphone or identify its owner. The driver also failed to produce a valid driver’s licence.

All three suspects were arrested for possession of a suspected stolen cellphone. The driver faces additional charges of reckless and negligent driving. The suspects were detained under Sunnyside SAPS Case 275/7/2026.

The swift and coordinated response by SAPS, supported by members of Axon Security Group, Proshield Security, Tactical Edge Security, Fidelity ADT, Hatfield CID and the Old East Safety Forum, was instrumental in the successful apprehension of the suspects.

Following the arrests, additional cases linked to Johannesburg and Olievenhoutbosch were also reported, and investigations are continuing to determine whether the suspects are connected to other cellphone theft incidents.

Incident Report
Subject: Fatal Incident Involving an Open Excavation
Date of Incident: 11 July 2026
Time of Incident: Approximately 20:21
Location: Corner of Lynnwood Road and Roper Street, Brooklyn, Pretoria

On Saturday evening, 11 July 2026, following the rugby match at Loftus Versfeld Stadium, a male pedestrian sustained fatal injuries after falling into an open excavation at the corner of Lynnwood Road and Roper Street, Brooklyn.

Following the conclusion of the match, a large volume of pedestrians was moving through the area. During this time, a man, believed to be middle-aged, fell into the excavation, hit his head on the bricks at the bottom, sustaining severe head injuries.

Bystanders, including off-duty medical doctors, nurses and trained first-aid responders, immediately entered the excavation to render emergency medical care and commence cardiopulmonary resuscitation (CPR). Advanced Life Support (ALS) paramedics arrived shortly thereafter and continued advanced resuscitation efforts. Despite these interventions, the patient succumbed to his injuries and was declared deceased at the scene.

During the rescue operation, three additional incidents were reported in the immediate vicinity:

▪️ Incident 2: An elderly male fell into the same excavation, sliding down the hole before coming to rest against an exposed drainpipe.

▪️ Incident 3: A young male bystander actively assisting with the initial rescue efforts slipped and fell into the same excavation.

▪️ Crowd Safety: A large number of bystanders gathered around the perimeter of the excavation. The weight and movement of the crowd caused sections of the edge of the excavation to collapse, resulting in loose soil falling onto emergency personnel while they were treating the patient. This created an additional safety risk for both responders and members of the public.

Scene Assessment

An assessment of the scene indicated that although there was some construction equipment, warning barriers and safety netting present in the general area, several deficiencies were observed at the incident:

▪️ Compromised safety netting: Multiple sections of safety netting had collapsed onto the ground and no longer served as an effective barrier.

▪️ Displaced barriers: Several physical barriers were observed to be unsecured and had been displaced from their intended positions.

▪️ Poor visibility: Limited public street lighting significantly reduced visibility for pedestrians moving through the area after dark.

▪️ Inconsistent site protection: Excavations closer to Loftus Versfeld Stadium appeared to have more effective protective measures in place. However, these measures became progressively less evident further along Lynnwood Road, with the excavation at the corner of Roper Street exhibiting inadequate protection for the volume of pedestrian traffic present after the event.

Operational Planning and Prior Observations

The Old East Safety Forum conducted a planned community operation in support of public safety during the rugby event. As part of our operational planning, we requested and reviewed the event's operational and traffic management plans.

Upon reviewing the documentation, it became apparent that the closure of Lynnwood Road and the associated roadworks were not reflected in their operational planning. This was further evidenced by the Springbok team convoy also having to divert through Brooklyn due to the road closure, indicating that the impact of the construction works had not been fully incorporated into the planned route.

These observations were communicated through informal channels to the Tshwane Metropolitan Police Department (TMPD) prior to the event. However, no revised operational plan addressing the roadworks or associated pedestrian safety concerns was subsequently received by the Old East Safety Forum.

As members of the Old East Safety Forum were already deployed in the vicinity as part of the planned community safety operation, they were able to respond immediately, together with members of the public and off-duty medical professionals, to render first aid and assist emergency services.

Earlier this afternoon, a truck collided with a tree in Menlo Park, bringing it down across a homeowner's driveway and damaging LPR cameras forming part of the Menloclose initiative.

CSI Security flagged the incident to the Old East Safety Forum, and our team wasted no time with chainsaws out, sleeves rolled up, and the driveway cleared in no time.

EMERGENCY SERVICES AND SECURITY MOBILISE AFTER ARCADIA COLLISION AND LOOTING

Incident Report: Motor Vehicle Accident
Date: 16 May 2026
Location: Intersection of Pretorius Street and Eastwood Street, Arcadia, Pretoria

Shortly after 08:10 on Saturday, 16 May 2026, multiple emergency medical services, law enforcement agencies, security companies, and towing services were dispatched to the intersection of Pretorius Street and Eastwood Street in Arcadia, Pretoria, following a motor vehicle accident involving a South African Breweries (SAB) truck and a Toyota Prado.

The circumstances surrounding the collision are yet to be determined, and investigations are ongoing. The South African Police Service (SAPS) is attending to the matter.

Two patients were assessed and treated on scene by attending emergency medical personnel. One patient, a minor, sustained moderate injuries and was transported to a nearby medical facility for further care. A second patient sustained minor injuries and was also transported to hospital.

Following the accident, members of the public began looting alcohol from the SAB truck. Additional resources were called to the scene to assist with crowd management and to prevent the continuation of looting. Law enforcement agencies and private security companies worked in collaboration to restore order and secure the area.

PEDESTRIAN VEHICLE ACCIDENT!

Just before 20:00 the Volunteer Emergency Medical Response Unit received a call for a pedestrian that was struck by a vehicle at high speed on the R21 highway close to the N1 interchange. On arrival with members from Pretoria Volunteer Emergency Services, one patient was found in the fast lane, on assessment the patient was found with extensive injuries and was declared deceased on arrival.

The scene was handed over to SAPS to investigate. We send our deepest condolences to the family of the deceased.

Incident Report: Pedestrian Vehicle Accident
Date: 28 April 2026
Location: Menlo Park

Shortly after 18:00 this evening, a pedestrian vehicle accident occurred in which a single male pedestrian was struck by a vehicle that subsequently fled the scene.

Members of the Old East Safety Forum responded and assisted with scene safety while the patient was assessed on scene by practitioners from PVES. The patient was thereafter transported by Gauteng Emergency Medical Services for further medical care.

Our thoughts are with the patient and his family, and we hope for a full and speedy recovery.

Incident Report: Motor Vehicle Accident
Date: 22 April 2026
Location: Francis Baard Street & N4, Koedoespoort

At approximately 06:20 this morning, a serious motor vehicle accident took place on the N4 East bridge, just past the Francis Baard Street onramp.

A taxi suffered a burst front tyre, causing the driver to lose control and collide with the side of the bridge. Tragically, the driver sustained fatal injuries and was declared deceased on scene. The incident also resulted in significant traffic congestion during the morning peak hour.

Members of the South African Police Service, including the SAPS Tshwane Accident Investigation Unit, attended the scene to conduct the necessary investigation. Volunteers from the Old East Safety Forum assisted with scene safety and traffic management throughout, and helped clear debris once the investigation had been concluded.

Our thoughts are with the family and loved ones of the deceased during this difficult time.

We urge all motorists to remain vigilant on our roads, maintain safe following distances, and ensure that their vehicles are roadworthy before setting out.
