PRESS STATEMENT | Health Tax Alliance, Rural Health Advocacy Project SA and TB Accountability Consortium welcome Finance Minister Enoch Godongwana’s continued focus on improving spending efficiency.

The organisations emphasise that efficient public spending must translate into better access to quality healthcare and improved patient outcomes, with adequate investment, accountability and effective resource allocation remaining essential to strengthening South Africa’s public health system.

The burden of preventable disease continues to grow, disproportionately affecting those in under-resourced and rural communities. A reactive system is no longer enough, we need one that actively reduces risk, strengthens primary care, and prioritises long-term public health outcomes.

taxes are not just fiscal tools, they are evidence-based interventions that can reduce harmful consumption while generating revenue to reinvest in the health system.

At the same time, strengthening district health services ensures that care is accessible, continuous, and responsive to the realities on the ground.

Speaking at the launch of South Africa's 2025 TIII launch, Rashaad Amra echoed a key message of the Health Tax Alliance: To***co excise taxes should be used first and foremost as a public health tool, not merely a source of government revenue.

While the to***co industry often highlights its economic contribution, the costs of to***co-related illness, lost productivity and pressure on the health system far outweigh the revenue generated from to***co taxes.

Stronger to***co control measures and health taxes remain critical to reducing consumption, preventing disease and protecting public health.

South Africa's To***co Industry Interference Index score stagnates at 64 in 2025, unchanged from 2023 and higher than in previous years, reflecting continued significant industry influence on public policy to increase to***co industry profits.

Presenting the 2025 findings, Lungile Mavundla highlighted the role of front groups and industry-linked organisations in shaping policy debates and resisting stronger to***co control measures.

Reflecting on the implications of the findings, Vimla Moodley, author of the SA To***co Industry Interference Index, warned that stability is not progress.

Five reports and years of recommendations later, South Africa remains trapped in a cycle of recurring vulnerabilities, weak transparency measures, and delayed to***co control reforms. The challenge, she noted, is no longer technical but political.

Their findings reinforce the need for stronger health taxes and to protect and safeguard public health and reduce to***co consumption.

The Index serves as an important resource for government, civil society, academia and public health advocates committed to advancing transparency, accountability and protecting public health in South Africa.
