Social, Health and Empowerment feminist collective of transgender women of Africa (SHE) has implemented on a program for “Investing for impact against Tuberculosis and HIV project” funded by the Global fund in Eastern Cape.

Purpose
To navigate clients through the referral process and ensure successful linkage to care:

Duties and responsibilities include but not limited to:
•Manage and support systematic tracking and monitoring of all referrals of clients.
•Tracking of client management information throughout client treatment period,
•Daily communication with health care facilities / stakeholders and program staff to ensure efficient and timely linkage to care and adherence support is provided to all clients.
•Develop and support efficient communication system between health facilities/ stakeholders and outreach teams.
•Support health care facilities and outreach teams to ensure contact tracing is being Implemented.
•Strengthen linkage to care for clients between DOH / stakeholders and outreach teams,
•Perform other duties as required.

Minimum requirement:
•Grade 12
•Minimum 1-3 year’s work experience in health and knowledge of HIV/AIDS, TB and STI
•Valid Code 08 Drivers licence will be an advantage
•Experience as a community health facilitator will be advantageous
•Possess good knowledge of HIV/AIDS prevention, care and treatment, and TB/HIV services

Other Skills

•Proficient personal computer skills
•Excellent attention to detail and deadlines
•Strong interpersonal skills and ability to build relationships with stakeholders and clients
•Good problem-solving ability
•Excellent written and verbal communication skills
•Very good reasoning ability.
•Be available to assist all HIV patients in the facility and from the community outreach sites

Purpose
To conduct surveillance for epidemiological investigation and contact tracing.

Duties and responsibilities include but not limited to:
• Be knowledgeable about health programs
• Manage and support systematic tracking and monitoring of all clients.
• Tracking of client management information throughout client treatment period,
• Daily communication with health care facilities / stakeholders and program staff to ensure efficient and timely adherence support is provided to all clients.
• Develop and support efficient communication system between health facilities/ stakeholders and outreach teams.
• Support health care facilities and outreach teams to ensure contact tracing is being implemented.

AIM
To reduce the number of new HIV infections and further spread of HIV through provision of recommended comprehensive package of services. Funded by the Global fund in Garden Route Western Cape. SHE seeks to employ Enrolled Staff Nurse to operate in George.

Purpose:
To provide a comprehensive prevention HIV package focusing on HTS and management of transgender (TG) programmes.
Duties and responsibilities
 Supervise the peer educators through their community engagement activities as well as during their HIV Testing Services.
 See to it that PE are testing according to the latest National HTS guidelines
 Ensuring all data tools and client information is kept safe and confidential by PE
 Provide technical support to PE on the ground, including create working relationship with facilities and introduce the programme to those facilities as well as introducing PE to new facilities to work with. Specify the support to the facility

 Ensure that PE have all necessary equipment that they need to do the work and that evidence of appropriate service provision is documented by PE and is properly complete
 Collect stock (condoms, IEC material and test kits) from M & E Officer, to give to the PE
 Ensure ongoing performance monitoring and mentoring / coaching (within the scope of work) to ensure that PE are competent to perform their duties
 Check all paper trial data is accurate and filled correctly during supervision
 Assist in providing education on health and lifestyle issues
 Coordinate the drop in centre and create a safe and friendly environment for client and provide service assistance
 Develop and maintain supportive and good working relationship with sub district, clinic supervisors and on cluster meetings etc.
 To make sure all equipment is packed in an orderly manner
 Supervisor and oversee all stock are kept in a controlled temperature (no exposure to sunshine or heat), ensure all equipment follows the quality assurance sop
 Monitor all stock and conduct stock checks,
 Monitor and oversee PE are providing quality care and services
 Ensure effective and efficient services and identify any gaps.
 Draft and facilitate in services training to correct any challenges around service delivery
 Assess client’s readiness for hormone replacement therapyThe candidate must fulfill the following requirements:

 Registration with the South African Nursing Council required
 A degree/Diploma in General Nursing
 Strong communication and interpersonal skills are essential
 Candidates must be available and willing to work flexible hours where necessary
 Have a drivers license.

 Understanding of sexual and gender diversity, knowledge of LGBTI issues would be an advantage.
