For over 15 years, CHIVA Africa has been equipping health professionals across South Africa with practical skills, knowledge and training in specialist child and adolescent healthcare. This has expanded beyond HIV to include young people with other diseases, such as TB, diabetes, epilepsy and mental health issues so they can protect and manage their own health as they reach adulthood. A young child living with HIV cannot manage their treatment alone.

They cannot travel to the clinic, collect their medication or understand why they must take it every day. They depend entirely on the caregiver beside them to keep them healthy.

But for caregivers living in deeply rural communities, providing this care can mean overcoming poverty, food insecurity, stigma, isolation and the high cost of travelling to a clinic.

With grant support secured through The Open University, CHIVA Africa has begun listening to caregivers of children living with HIV. Some interviews took place under a tree or inside our vehicle—but each created a safe space for caregivers to share experiences that are too often unheard.

We met families who rely on a river for water, subsistence farming and a child support grant of approximately R580 a month. Although a clinic may be only eight kms away, a return taxi journey can cost R60 - money that is also desperately needed for food and other essentials.

Behind every fluctuating viral load is not simply a number. There is a vulnerable child whose future depends on a caregiver often doing everything possible in extraordinarily difficult circumstances.

Every caregiver we met was grateful for the opportunity to speak openly and be heard without fear or judgement. Now, we must turn their stories into practical, caregiver-led support that helps children remain healthy and thrive.

Recent headlines about South Africa and the US funding for HIV have focused heavily on politics.

But beyond the noise lies a quieter crisis with potentially devastating consequences: the weakening of HIV health systems due to reduced funding.

For children and adolescents living with HIV, this matters deeply.
The greatest threat is often not the lack of medication itself, but the breakdown of the systems around it—trained healthcare workers, viral load monitoring, early infant diagnosis, caregiver support, tracing, and psychosocial care.

When these systems weaken, children are missed.
Missed tests become missed diagnoses.
Missed appointments become treatment failure.
Treatment failure can become loss of life.

Children living with HIV are among the most vulnerable because they depend entirely on adults and functioning systems to keep them healthy.

South Africa has made remarkable progress in HIV care, but these gains remain fragile.

Now is not the time to reduce investment.

Now is the time to strengthen systems, support healthcare workers, and protect the most vulnerable.

Because when systems fail, children pay the price.

South Africa’s HIV programme is facing new challenges, with recent reports showing slower uptake of life-saving antiretroviral treatment and growing gaps in support for vulnerable communities.

Children exposed to and living with HIV cannot be left behind.

CHIVA Africa is committed to strengthening healthcare systems and supporting frontline healthcare workers to better respond to the needs of vulnerable children and families. Earlier this year, our team trained and mentored 144 healthcare workers in the iLembe District to improve the use of HIV monitoring tools and strengthen patient follow-up and care.

Every child deserves the opportunity to thrive. Every family deserves support. Every healthcare worker deserves mentorship and guidance.

Together, we can continue working towards zero new HIV infections, zero AIDS-related deaths, and healthier futures for all children.

For mothers living with HIV, staying on treatment and achieving viral suppression can make breastfeeding much safer, with the risk of HIV transmission becoming extremely low when viral load remains suppressed.

Healthcare workers play a critical role in counselling and supporting mothers through this journey — helping them make informed choices that protect both mother and baby.

When a person living with HIV is on consistent antiretroviral treatment and has an undetectable viral load, they cannot transmit HIV to a sexual partner.

Undetectable = Untransmittable (U=U)
