PROFESSIONAL NURSE / ENROLLED NURSE
COMMUNITY BASED HEALTH SERVICES COORDINATOR / BRANCH OUTREACH TEAM LEADER

LIVING HOPE is a Christian non-profit organisation that strives to Bring Hope, Break Despair and Build Africa.
Living Hope seeks to appoint a CBHS Coordinator / Branch OTL to be based in Masiphumelele, Cape Town. Living Hope provides services across multiple provinces in South Africa and is committed to strengthening community-based healthcare services nationally.
The goal of Community Based Health Services (CBHS) is to prevent unnecessary visits to health facilities through high-quality and appropriate care that helps clients maintain their independence and achieve the best possible quality of life.
The successful candidate will be responsible for the assessment of clients, development and monitoring of care plans, allocation of clients and daily work schedules, regular follow-up visits, record keeping, networking with local clinics, community health centres and hospitals, and the ongoing training, supervision and support of Community Health Workers.
Requirements for the Position
· Professional Nurse (PN) or Enrolled Nurse (EN)
· Current SANC Registration
· Ability to work independently in community settings
· Ability to supervise and coordinate Community Health Workers
· Good communication skills and fluency in English and at least one additional local language
· Strong administration skills
· Strong planning, organisational and time management skills
· Ability to solve problems in resource-constrained environments
· Cultural sensitivity and ability to work within diverse communities
· Valid driver's licence with at least two years' driving experience
· Basic computer literacy
· Desire to serve God in a meaningful and practical way
