Lockout/Tagout/Tryout (LOTO) safety procedure used in industrial and maintenance settings to prevent accidental machine startup or energy release.

Lock Out (LO) – A physical lock is placed on the energy source (such as a switch or valve) to prevent it from being turned on. This ensures that the equipment remains inoperable until the lock is removed by an authorized person.

Tag Out (TO) – A warning tag is attached to the lockout device, indicating that the equipment must not be operated. The tag provides additional information such as the person responsible for the lockout and the reason for it.

Try Out (TO) – After locking and tagging the equipment, an attempt is made to turn it on to verify that the lockout is effective. This step ensures that no residual or stored energy remains that could cause harm.

This LOTO procedure is essential for workplace safety, preventing accidents and injuries during equipment maintenance.

"As seen in the video, the heavy lifting practice presented is extremely poor."

Let's assess this accident together:

1. Failure to establish the Center of Gravity.
2. Webbing sling in a choker position at a sharp angle (Not allowed for lifting in the lifting operations).
3. Absence of a lifting plan for this lifting operation.
4. Risk assessment/analysis not conducted, as evidenced by the lack of barricades in the lifting area.
5. The safe lifting procedures not implemented during lifting operation.
6. Worker are observed not wearing PPE as required.
7. No pre-inspection of lifting gear before use.
8. Inexperienced rigging supervisor.

What is a Safe System of Work?
A Safe System of Work (SSOW) is a structured approach to identifying and eliminating hazards during tasks. If elimination isn't feasible, it minimizes risks through safety measures.

When is a Safe System of Work Needed?
SSoWs are required when hazards can't be physically eliminated,

such as during:
Equipment cleaning and maintenance.
Changes in work layouts or methods.
Emergency repairs and breakdowns.
Managing contractor activities.
Material handling operations.

Steps to Establish a Safe System of Work:

01. Assess the Task:
A trained individual evaluates tasks, considering materials, equipment, environmental factors, and emergency protocols.

02. Identify Hazards:
Identify hazards and aim for elimination. If not possible, reduce risks via substitution, engineering controls, administrative measures, or PPE.

03. Formulate Safe Methods:
Develop written procedures detailing safe methods, including pre-task preparations, operational plans, access routes, and waste disposal. For high-risk tasks, implement a "Permit-to-Work" system.

04. Implement the System:
Communicate procedures effectively. Supervisors must ensure employees understand risks, provide training, and oversee compliance.

05. Monitor the System:
Regularly check practicality, adherence, and effectiveness.
Update the system for changes in circumstances, incidents, or new technologies.

Using the 𝗧𝗜𝗟𝗘 (Task, Individual, Load, Environment) method, you can evaluate risks and develop safer handling practices. Let’s break down these four factors:

🔷 𝗧𝗮𝘀𝗸: The nature of the task significantly impacts the risk of injury. Consider:
- How far and how often the load needs to be moved
- Whether the task involves awkward postures, bending, twisting, or reaching
- Duration of the task—repeated or prolonged movements increase risk

🔷 𝗜𝗻𝗱𝗶𝘃𝗶𝗱𝘂𝗮𝗹: Each worker's physical capability must be assessed.
- Age, strength, and fitness levels play a role in their ability to handle loads safely
- Training and experience in proper lifting techniques are vital
- Pre-existing health conditions, particularly musculoskeletal issues, should be factored in

🔷 𝗟𝗼𝗮𝗱: The characteristics of the object being handled are key to risk evaluation.
- Weight, shape, and size of the load—heavier or bulkier items present higher risks
- If the load is difficult to grasp, slippery, or has uneven weight distribution, injury risk increases
- Hazardous substances or sharp edges on the load also pose additional risks
