PACE® was created as a way to provide you, your family, caregivers, and professional health care providers, flexibility to meet your unique health care needs while helping you to continue living in the community. An interdisciplinary team of healthcare professionals will give you the coordinated care you need. These professionals are trained and experienced in working with older people. They will
work together with you and your family (if appropriate) to develop the most effective plan of care for you. PACE® provides all the care and services covered by Medicare and Medicaid, as authorized by the interdisciplinary team, as well as additional medically-necessary care and services not covered by Medicare and Medicaid. PACE® provides coverage for prescription drugs, doctor care, medical transportation, home care, check-ups, hospital visits, and even nursing home stays whenever necessary. A participant may be fully liable for the costs of medical services from an out-of-network provider, or without prior authorization (except in emergency situations). Huron Valley PACE is the sixth PACE program in the State of Michigan and there are 102 across the United States. Further information on the PACE Program can be found at npaonline.org. In order to join the Huron Valley PACE® program an individual must:
Be age 55 or older
Be certified as meeting the State of Michigan criteria for nursing facility level-of-care
Live within the Huron Valley PACE® service area
Be able to live safely in the community with the support of Huron Valley PACE®
