🚨 ATTENTION YAKIMA! KNOW YOUR RIGHTS WHEN CHOOSING AN FCS PROVIDER! 🚨

If you receive Foundational Community Supports (FCS) through Washington Apple Health, you have an important right that everyone should know about:

👉 YOU have a choice in who provides your FCS services.

FCS is designed around person-centered care, participant choice, and voluntary services. You should have a provider that works for you — not the other way around.

🏠 Looking for an FCS provider?

Washington has multiple FCS providers offering Supportive Housing and Supported Employment services. You can use Wellpoint’s FCS provider search to explore available providers and find one that fits your needs.

🔹 You can ask questions about different providers.
🔹 You can choose a provider that meets your needs and preferences.
🔹 You can change your FCS provider if your current provider isn’t working for you.
🔹 You are not required to stay with a provider simply because they helped you find housing.

Wellpoint’s current FCS Provider Change Request specifically states that it is the enrollee’s choice to change their FCS provider and that an enrollee is not required to work with a specific provider simply because that provider has housing.

⚠️ One important thing to understand

You can be enrolled with ONE FCS provider at a time for your FCS services.

However, FCS and HEN are separate programs. Being enrolled with an FCS provider does not mean you cannot receive HEN services from a different provider, when you are eligible for those services.

Most importantly:

No provider should pressure, threaten, or manipulate you into staying with them. Your FCS services are YOUR services, and your choice matters.

If you feel like you were not given a choice of FCS provider, or you have questions about changing providers, you can contact Wellpoint FCS directly at:

What an incredible day for our community! ❤️

Consistent Care Support Services would like to send a HUGE THANK YOU to everyone who helped make our Back-to-School event such a success. Because of the generosity and support of our community, we were able to help provide school supplies to more than 200 kids! 🎉📚

🏗️ A special thank you to Joyce Construction, Yakima County Sheriff’s Office Foundation, and Yakima Valley Grid Kids Viper team for your generous donations and for helping us make sure local students have the supplies they need to start the school year ready to succeed.

🤝 Thank you to Peers Rising, United Way of Central Washington, Triumph, Yakima Union Gospel Mission, Rods house, and Catholic Charities who donated their time, showed up for our families, and helped make the event special. Your continued partnership means more than you know!

💈✂️ And a HUGE shoutout to Luiz Rodriguez Professional Real Estate Broker with Royalty Barber Studio for generously donating his time and talent to provide haircuts for kids! There is nothing like heading into a new school year with fresh supplies, a fresh haircut, and some extra confidence! 🙌

🌟 CLIENT SUCCESS SPOTLIGHT: SHAWN 🌟
From treatment ➡️ leadership ➡️ a career helping others ➡️ his own home.

Today, Consistent Care Support Services is proud to celebrate Shawn and the incredible journey he has built through recovery, determination, and a commitment to giving back.

After completing treatment, Shawn made the decision to continue strengthening his recovery by moving into clean and sober housing. He didn’t just focus on his own success—he quickly became a leader and positive example to those around him. His hard work and dependability led to him becoming a House Manager, supporting fellow residents and helping create a safe and supportive environment for others in recovery.

But Shawn wasn’t finished. 💪
Through his own experiences, Shawn discovered a passion for helping people facing challenges similar to those he had overcome. He completed Peer Support training and turned that passion into a career. Today, Shawn is employed full-time as a Peer Support Specialist and serves on the Rapid Response SPRINT Team, helping connect individuals with support following an opioid overdose.

Using his compassion and lived experience, Shawn now helps others find treatment, recovery services, housing, and community resources, often meeting people during some of the most difficult moments of their lives and showing them that recovery and a different future are possible. ❤️

🏠 Then came another huge milestone: his own apartment!

Working alongside his Consistent Care case manager, Shawn searched for housing that was safe, sustainable, and within his budget. He found the right apartment, signed his lease, and officially transitioned into independent living.
And his dedication continues to be recognized. Shawn was recently honored by his employer for his outstanding hard work, commitment, and dedication to helping others on their recovery journeys. 🏆

Shawn’s story represents what success can look like when determination, opportunity, recovery, housing, and employment come together.

Treatment became recovery.
�Recovery became leadership.
�Leadership became a career.
�And that career became an opportunity to help others change their lives.

👏 Congratulations, Shawn!
