This outreach program is doing exactly what community care should look like, meeting people where they are, building trust, and making sure vulnerable residents aren’t left to navigate things alone. And it’s made possible in part through the North Central Community Care Hub (CCH), a collaborative effort that Action Health Partners helps lead.

This kind of coordinated effort matters. It connects local providers, strengthens response systems, and helps ensure people can access the support they need, when and where they need it.
