The Health Resources and Service Administration has put a proposal for a change in formula funding to Part A jurisdictions that would adjust resource allocations from place of diagnosis to where you live and receive care. The proposal was put in the Federal Register on November 10 and is currently open for public comment which closes December 10, 2025. CAEAR will be holding a community zoom call to discuss the implications of the proposal on December 2, to decide whether CAEAR will take an organizational position. The HIV service provider community has long raised concerns about the fairness of jurisdictions getting resources based on diagnosis rather than provision of care so the HRSA study is welcome, and responsive to community requests. However, CAEAR has not been briefed by HRSA on the methodology of the study. CAEAR is interested in the time period studied and the extent to which external but temporary factors were taken into consideration. Will the residence of individuals every June 30th be the reference or will a longer period to tim be considered? We know that everyone diagnosed is not in care and not all of those in care are in the Ryan White system. How will those issues be taken into consideration? HIV/AIDS is a communicable disease and as such it has been a public health priority to test and treat as many people as possible within our jurisdictions. Will that continue to be a public health priority in the pursuit of controlling and ultimately ending the HIV pandemic? The HASA proposal raises many important questions that historically would have been discussed thoroughly before a federal register notice was issued of this significance. We look forward to CAEAR’s community call to discuss the implications of the proposed federal policy and funding changes.

This is NOT a drill -- if you haven’t activated your networks yet, please do so IMMEDIATELY! This includes your work networks, coalition partners, parent networks, community orgs, health care providers, health centers, hospitals and everyone who depends on Medicaid. Rope in your personal networks -- family, friends, etc. via social media. We need LOTS of calls to Congress ASAP!!!!!

As you know, the House Budget Committee has approved a budget resolution that includes a minimum of $880 Billion in cuts to Medicaid. The House is on recess this week but we expect that the budget resolution will be voted on next week. While Trump gave lip service to saying Medicaid wouldn’t be cut, today he endorsed the House’s budget resolution bill. So we need to make a concerted effort NOW to stop the House from passing this resolution. Once the House & Senate agree on a joint budget resolution, the relevant committees must come up with the cuts. Members will be under a lot of pressure to vote for the budget resolution to keep the process moving. But if the resolution includes $880 Billion in cuts, it will be much more difficult to stop farther down the line.

The CAEAR Coalition mourns the passing of our dear friend and colleague Carlton Smith today. Carlton represented the Baltimore Part A EMA with distinction as its Planning Council Chair, and community liaison. Despite a variety of health challenges including HIV, he was always ready to lead and support his colleagues and individuals living with HIV. We will miss his playful spirit, commitment, and myriad contributions to our work.

CAEAR mourns the death of former Senator Mike Enzi (R-WY), longtime member and former chair of the Senate Health, Education,Labor and Pension (HELP) Committee. He and his staff worked closely with CAEAR leadership and our members to reauthorize the Ryan White program on each occasion it was passed. He was a strong proponent of local community control, and treated the issues of people living with HIV with great empathy and respect. We honor his public service, and send our condolences to his family and the state of Wyoming he represented for many years.

The genial, low-key conservative served in the Senate for four terms and was chairman of two committees critical to domestic policy.

CAEAR Congratulates Harold Phillips, COO of the End the Epidemic Plan for America at HHS on his appointment as the Director of the White House Office of National AIDS Policy. As a long time leader at HRSA working on the Ryan White HIV Treatment Program, and Global HIV, Harold is well known to the CAEAR community. We look forward to a productive relationship with him in his new role, and are excited by the elevation of the End the Epidemic Plan to the White House providing it with the cross departmental cooperation and oversight it has needed from its inception.

How remarkable! Despite the challenges many of our public health systems continue to face, our Ryan White systems of care have achieved this result. It speaks to the steadfast commitment of our planning and service delivery communities that continue to review, refine, and respond to the needs of people living with HIV. CAEAR couldn’t be prouder of our efforts to ensure this law survives and thrives across the U.S. as a model of care to be replicated by other components of our health services systems. Job well done! The Health Resources and Services Administration's (HRSA) HIV/AIDS Bureau (HAB) released three Notices of Funding Opportunity (NOFO) for the new fiscal year (FY) 2020 with March 1, 2020 start dates:

· Ending The HIV Epidemic: A Plan For America – Ryan White HIV/AIDS Program Parts A And B (HRSA-20-078)
· Ending The HIV Epidemic: A Plan For America – Technical Assistance Provider (HRSA-20-079)
· Ending The HIV Epidemic: A Plan For America – Systems Coordination Provider (HRSA-20-089)
Ending the HIV Epidemic: A Plan for America – Ryan White HIV/AIDS Program Parts A and B (HRSA-20-078)
As part of this funding opportunity, HRSA plans to fund up to 47 awards to ultimately reduce new HIV infections in the United States to less than 3,000 per year by 2030.

The anticipated funding amount is $55.1 million in the first year of the five-year period of performance to implement strategies, interventions, approaches, and core medical and support services to reduce new HIV infections in the United States. The Ending the HIV Epidemic initiative will focus resources in 48 counties, Washington, D.C., San Juan, Puerto Rico, and seven states with substantial rural HIV burden.

Applicants eligible for this funding opportunity are:

· RWHAP Part A funded Eligible Metropolitan Areas (EMAs) or Transitional Grant Areas (TGAs) whose service area includes one or more of the identified 48 HIV high burden jurisdictions.

· RWHAP Part B funded states identified as having a substantial rural HIV burden–Alabama, Arkansas, Kentucky, Mississippi, Missouri, Oklahoma, and South Carolina.

· The RWHAP Part B Program of the State of Ohio on behalf of Hamilton County.

HRSA's HIV/AIDS Bureau will host a technical assistance webinar for applicants on August 27, 2019, at 2 p.m. ET. Please reference the NOFO for the dial-in and webinar recording information.

Ending the HIV Epidemic: A Plan for America – Technical Assistance Provider (HRSA-20-079)
HRSA plans to fund one organization to provide technical assistance to recipients of Ending the HIV Epidemic: A Plan for America- Ryan White HIV/AIDS Program Parts A and B (HRSA-20-078) on implementation of work plan activities, innovative approaches, and interventions.
Applicants eligible for this funding opportunity are public and nonprofit entities, including institutions of higher education and academic health science centers involved in addressing HIV related issues on a national scope. Faith-based and community-based organizations, Tribes, and tribal organizations also are eligible to apply.

Ending the HIV Epidemic: A Plan for America – Systems Coordination Provider (HRSA-20-089)
HRSA plans to fund one organization to assist recipients of Ending the HIV Epidemic: A Plan for America- Ryan White HIV/AIDS Program Parts A and B (HRSA-20-078) in coordinating and integrating their initiative plans, funding sources, and programs with the existing HIV care delivery systems. In addition, the Systems Coordination Provider will assist in identifying existing and new stakeholders, as well as collate and disseminate best practices, innovative approaches, and interventions identified by the Technical Assistance Provider that will advance recipient’s progress in meeting the goals of the initiative.
Applicants eligible for this funding opportunity are public and nonprofit entities, including institutions of higher education and academic health science centers involved in addressing HIV related issues on a national scope. Faith-based and community-based organizations, Tribes, and tribal organizations also are eligible to apply.
