The Society of Interventional Oncology (SIO) has submitted updates to the NCCN Neuroendocrine and Adrenal Tumors Guidelines, aimed at modernizing liver-directed therapy recommendations. Authored Rony Avritscher, MD, UT MD Anderson Cancer Center; Nicholas Fidelman, MD, University of California San Francisco; and Etay Ziv, MD, PhD, Memorial Sloan Kettering Cancer Center, requested changes include expanding ablation language to incorporate irreversible electroporation (IRE), recognizing equivalent efficacy between TAE and conventional TACE, and softening recommendations regarding octreotide prophylaxis and overnight observation to better reflect current clinical practice.

SIO also proposes new guidance for optimizing TARE dosing strategies, consideration of TARE for ATRX/DAXX-mutated tumors, and correction of a minor terminology error. Overall, the recommendations seek to align NCCN guidance with emerging evidence, evolving interventional oncology techniques, and contemporary patient management practices.

The Society of Interventional Oncology (SIO) has submitted updates to the NCCN Adult Cancer Pain Guidelines, requesting that the current "Interventional Strategies" section be renamed "Principles of Interventional Oncology" and expanded to include updated guidance on patient selection and image-guided tumor ablation for painful bone metastases and locally advanced bone tumors.

SIO has submitted updates to the NCCN Breast Cancer Guidelines, requesting the recommendation to allow percutaneous cryoablation without surgical excision as an alternative local therapy for patients with low-risk, early-stage breast cancer. The petition cites the FDA’s authorization of a breast-cancer-specific cryoablation device, endorsement from the American Society of Breast Surgeons, and prospective clinical trial data demonstrating low recurrence rates and favorable safety, arguing that cryoablation offers a patient-centered, less invasive option.
