Active Re-Entry Uintah Basin Active Re-Entry is a community based program which assists individuals with disabilities to achieve o If you qualify, the devices are free.

Active Re-Entry covers 27,000 square miles fo Eastern Utah and our services and programs are free. Some of our Assistive Technology services - such as obtaining an electric scooter or other assistive technology - requires an eligibility and financial intake process. Also ask about our equipment loan bank, which is open to everyone.

Hey Uintah Basin!! Active Re-Entry has two durable medical equipment loan banks available in your area. Items available are wheelchairs, walkers, shower chairs, toilet risers, grab bars, canes, knee scooters. If you need these items for a day or a year we can help. Contact Jamie 435-789-4020 if your in Uintah or Daggett counties or Ryan 435-630-0500 if your in Duchesne county.
