SESSION SPOTLIGHT 2 — EMERGENCY DEPARTMENT

Few places make problems with flow as visible as the Emergency Department.

Waiting rooms fill.

Patients board while waiting for beds.

Staff, diagnostic resources and capacity are pulled in multiple directions.

But where is the actual bottleneck?

At TOC + Healthcare 2026, Dr. David Boger II and Dr. Benjamin Newman will explore how Theory of Constraints can be applied in the Emergency Department to better understand these competing demands.

The session will look at common ED bottlenecks and how TOC principles can help teams improve flow, clarify priorities and focus improvement where it can make the greatest difference.

It is not simply about making one part of the ED faster.

It is about understanding what is limiting the performance of the system.

The Emergency Department provides the example, but the challenge exists across industries. Any organization managing limited capacity, changing demand, competing priorities and interconnected resources can apply the same thinking to identify what is truly constraining flow and where improvement should be focused.

Are you solving the problem or just the symptom?

✅ A long wait.
✅ An overloaded department.
✅ A capacity shortage.
✅ A delayed project.
✅ A growing backlog.

These are the problems we can see.

But adding capacity, working harder or fixing the place where the symptom appears does not necessarily improve the performance of the whole system.

The harder—and more valuable—question is:

What is really limiting performance?

At TOC + Healthcare 2026, we will explore how to move beyond visible symptoms and better understand the system behind them.

Healthcare provides the examples, but the challenge is universal.

Whether you are managing patients, projects, production, supply chains or services, sustainable improvement begins by understanding:

➡️ Where the real bottleneck is
➡️ What is causing it
➡️ Where to focus your efforts

Don’t just fix what hurts. Learn how to find what is really holding the system back.

You can have the right solution and still struggle to get the result.

Why?

Because the solution itself may not be the bottleneck.

The real constraint may be an untested assumption, an unresolved dependency, an overlooked obstacle, or a necessary condition for success that is not yet in place.

On October 28, Thorsteinn Siglaugsson will share a real-world case from Statistics Iceland, where the Logical Thinking Process was used to prepare for the implementation of a new standardized organizational data-flow.

Using the:

Goal Tree → Future Reality Tree → Prerequisite Trees

the team worked to clarify the goal, test the proposed future state, and identify what could prevent the solution from delivering the intended result.

What if you could see what's really holding you back?

When performance stalls, the most obvious problem isn't always the real problem.

Bottlenecks aren't always easy to identify. And understanding the cause-and-effect relationships behind persistent challenges can be even harder.

What if you could make those relationships visible?

Meet Flying Logic, a visual thinking tool that helps individuals and teams:

➡️ Look beyond symptoms to understand why problems persist.
➡️ Explore what's limiting performance and uncover potential bottlenecks.
➡️ Challenge assumptions and identify opportunities for improvement.
➡️ Turn insights into action with a clearer path forward.

Whether you're familiar with Theory of Constraints or simply looking for a better way to solve complex problems, Flying Logic can help bring clarity to your thinking.

What happens when the system you are trying to improve is YOU?

What if the same thinking we use to find bottlenecks in organizations could help us understand what is limiting personal change?

Join Dr. Lisa Ferguson for "My 90-Day Human Transformation Project" - an exploration of what happened when she applied TOC-based thinking to a 90-day transformation focused on mental, emotional, physical and spiritual well-being.

The focus is not on Lisa’s personal journey, but on the transferable thinking and methods others can apply to personal goals, meaningful work, health and life design.

Find what is limiting progress. Focus where it matters. Create meaningful change.

SESSION SPOTLIGHT:

Where do you focus when everything feels important?

Healthcare teams are managing rising demand, limited capacity and competing priorities across systems where one decision can affect many others.

The opening session of TOC + Healthcare 2026 goes back to the foundations.

TOC in Healthcare: Flow, Constraints & the Five Focusing Steps will introduce how Theory of Constraints can help teams look at the whole system, identify what is really limiting performance and determine where improvement should begin.

Rather than trying to improve everything at once, the Five Focusing Steps provide a structured way to identify the constraint and focus effort where it can have the greatest system-wide impact.

Healthcare provides the setting for this session, but the thinking applies much more broadly. Any organization working with limited capacity, competing priorities, dependencies or bottlenecks can use the same approach to better understand where to focus and how to improve the performance of the whole system.

What if delaying supplier payments is actually slowing down your own cash collection?

This Wednesday, Ravi Gilani explores the connection between supplier confidence, material availability, production flow, OTIF performance and how quickly customers ultimately pay.

The Fastest Way to Get Paid: Pay Fast to Collect Fast challenges the assumption that holding onto vendor payments always improves working capital.

Instead, the session asks:

Where is the bottleneck in the journey from supplier payment to cash in the bank?

Long waits. Delayed discharges. Capacity pressure. Overloaded teams.

These problems are easy to see.

But they may only be symptoms of something deeper.

What is actually limiting the system—and what should change first?

On October 20, join Danilo Sirias, Ph.D., TOCIC and Dr. Christopher Strear, MD, MBA for a live introductory session exploring how Theory of Constraints can help healthcare leaders and improvement professionals look beyond visible symptoms, identify what is really limiting flow, and focus improvement efforts where they can have the greatest impact.

The discussion will connect these ideas across emergency care, inpatient services, operating rooms, outpatient care and behavioral health.
