There are so many pieces to recovery and often patients need help getting connected. relink.org is used by caseworkers to help those being released find the resources they need. Often, they are walking alongside a person that may be recovering from an illness or an injury where they have been in the hospital for a long time. They can help their clients find food pantries, transportation, and many more resources that might help them when they are released.
