National Academy of Certified Care Managers - NACCM The National Academy of Certified Care Managers (NACCM) is a nonprofit organization providing the premier certification for Care Managers since 1995.

Welcome to NACCM, a nonprofit organization, providing the premier certification for Care Managers since 1995. A Care Manager is a health and human services specialist who acts as a guide and advocate for clients and/or families who are caring for older relatives or disabled adults. NACCM assures that Care Managers are qualified to perform the full range of care management tasks through a validated

, standardized examination. Consumers, employers, and other professionals can be confident in their choice when selecting a Care Manager who hold the CMC credential. All CMC Candidates must meet rigorous education, experience, and supervision requirements to qualify for the exam. Candidates are tested on the five domains unique to care management. The test is administered by an independent testing company. In order to maintain certification, CMCs are required to participate in continuing education and professional development. Certification is renewed every three years to ensure that Care Managers are practicing at the highest professional level.

Lisa Robins, RN, BSN, CMC, is the founder of Hand in Hand Care, where she provides personalized care management services to older adults throughout Sarasota and the surrounding communities. A Sarasota native and cm laude graduate of the State College of Florida, Lisa combines her clinical expertise as a Registered Nurse with a deep commitment to patient advocacy, helping older adults age in place with dignity, safety, and confidence.

Lisa specializes in supporting older adults living with dementia, limited mobility, and complex medical needs. Her community-centered approach focuses on coordinating care, advocating for clients, and ensuring they receive the skilled support they need while remaining in the comfort of their own homes.

Inspired by both her professional and personal experiences, Lisa founded Hand in Hand Care after recognizing a critical gap in senior healthcare. While working in acute rehabilitation, she witnessed many patients make significant progress only to return to the hospital because they lacked coordinated support after discharge. At the same time, caring for her mother during her battle with stage IV breast cancer gave Lisa firsthand insight into the challenges families face while navigating complex medical care. These experiences reinforced her belief that older adults and their loved ones deserve an experienced advocate to guide them through every stage of the care journey.

For Lisa, earning the Care Manager Certified (CMC) credential reflects her commitment to professional excellence and lifelong learning. She believes certification demonstrates to clients that they are working with a qualified, dedicated professional while helping her continually strengthen her skills and grow her practice.
