"I am honored to nominate Alex for recognition for the meaningful impact she has made during her nearly four years as our MDS Coordinator. While Alex plays an essential role in care planning, she is so much more than a โ€œcare plan driver.โ€ She takes the time to truly know our residents and build genuine relationships with them and staff. Alex knows our residents so well that during IDT discussions, she is often the first to say, โ€œNo, that isnโ€™t what they like.โ€ And 99.9% of the time, she is right. To understand Sarah, you should know she is the proud mother of two children and will admit she is a โ€œhelicopter mom.โ€ That same loving and protective nature carries into her work, making her our โ€œhelicopter LBSW.โ€ Sarah watches over our residents, stays involved, follows up, asks the hard questions, and makes sure no oneโ€™s needs fall through the cracks.

Sarah goes above and beyond to help residents and families through the emotional and often overwhelming transition into assisted living or long-term care. She listens, provides honest answers, explains what to expect, and offers reassurance throughout the process. She treats every resident and family as if they were her own.

Her dedication also extends beyond our facility. Sarah helps community members complete Medicaid applications and guides families through complicated financial and healthcare processes. She patiently walks them through each step and never makes anyone feel like their questions are a burden.

Sarah is a strong advocate for resident rights, dignity, choice, and quality of life. She works closely with families, staff, providers, and outside agencies to find solutions and support the whole person.

Whether she is watching over her own children or our residents, Sarah gives her whole heart. Winfield Senior Living Community is better because of her compassion, experience, humor, and dedication."

"Some people find a job; others find a calling - this nominee has found both. She began her career as a CNA, where she learned the power of small acts of kindness. When her facility needed a CMA, she stepped forward to become one. Later, she eagerly accepted the opportunity to become Activity Director, and she has been enriching lives ever since. Her creativity, determination and dedication have transformed daily life for residents. She doesn't ask, "Can we do this?" she asks "How can we do this?". Knowing many residents could no longer make the trips to locations she brought locations to them. She recreated the Precious Moments Chapel inside the facility. Residents have also enjoyed virtual trips to Silver Dollar City (yes, even roller coaster rides), music, cooking experiences, art projects, and imaginative themed events. Every activity is thoughtfully planned and every one of those activities is laced with laughter, smiles, and memories. We have happy residents! What truly sets her apart is her positivity. I don't believe this young woman knows how to frown. She is always smiling, always encouraging, always willing to help. Her joy is contagious - and so is her giggle. She has set high goals for herself because this is not just a job. Though early in her career she has demonstrated leadership, wisdom, and compassion beyond her years. She wants to leave a lasting impact on the profession and on the lives of those she serves. She reminds us that activities create purpose, preserve dignity, spark, joy, and build connections. "Jay is our Director of Financial Services but serves in several other capacities. Jay does a great job of controlling our communities' expenses and oversees the billing process but also serves as our unofficial onsite IT technician which I know all communities can appreciate. One of the duties that Jay has taken upon himself to be in charge of is the upkeep of our downtime computer. We recently were forecasted to have bad storms, Jay reached out to our DON asking if she felt the need to print off all the MARs and TARs just in case we lost internet service or power. Without even receiving a response, Jay quickly printed off all the MARs and TARs for both our AL and SNF residents organized them all in binders and dispersed them out to all the units. We luckily did not have to use them, but this is one of the many examples of how Jay helps support all aspects of our community. He is knowledgeable of all the residents and families and is one of the main staples at Rosehill!"

"Adult care has been a part of my nursing career for 20 years. Over those years, I have watched so much evolve and change in long-term care, it is always heartwarming when you can work alongside a CMA who shows compassion, dedication, and genuine care for the people we serve.

Debra is the kind of CMA who understands that caring for residents is about so much more than completing tasks. She has a wonderful way of making residents feel seen, heard, and valued, she treats every resident as a human being first.

One of the things that makes Debra especially valuable is the way she helps new CNAs learn not only how to do the job, but how to care for residents as individuals. She leads by example and shows that being a good caregiver is about seeing the person.

She also encourages residents to do for themselves whenever they are able, helping them maintain their highest level of independence and function.
