You WILL need an updated physical to participate in Flag, Padded Flag, and Tackle Football Leagues. If your physician's office does not have a physical form, you may print one from the link below.

PLEASE, make sure your athlete's physical is complete before the first day of practice. Athletes WILL NOT be able to participate until the updated physical is on file.
