DO YOU WANT TO HAVE A VOICE! Calling Florida Cancer Survivors, Caregivers and Community Members
Moffitt Cancer Center is currently conducting a Community Health Needs Assessment as mandated by the Affordable Care Act & IRS for non-profit hospitals. One component of assessing the health needs is to solicit the input of community members. This is an integral step to ensuring we are able to understand the needs of our community and develop programs and services that will positively impact the health and well-being of those we serve.
We are asking you to take the survey and then forward this email to everyone in your sphere of influence including colleagues, clients, partners, employees and family members. You will be asked questions related to demographics, your personal health behaviors and beliefs, accessing healthcare, and perceptions of your community.
The survey is for adults ages 18 and older who live in Moffitt’s 15-county catchment area: Hillsborough, Pinellas, Pasco, Polk, Sarasota, Manatee, Hernando, Citrus, Lee, Charlotte, Highlands, Sumter, Lake, Hardee, Desoto Counties. Your responses are anonymous and confidential. Your participation should take no longer than 20-25 minutes and is completely voluntary.
There may be no direct benefit or compensation to you for your participation; however the information will benefit Tampa Bay residents. A summary of the information you provide will be used to develop a plan to address the most important health issues that are identified.
The Community Health Needs Assessment report will be published on www.Moffitt.org in Summer 2019.
