The Polk HealthCare Plan (PHP) may be a solution. PHP offers health coverage for full-time Polk County residents who are uninsured and receive little or no income.

At The Family Healthcare Foundation, we can help you determine whether you qualify.

To determine your eligibility for the Polk HealthCare Plan, you will need:

💙 Your Social Security number
💙 Documentation showing any sources of income

Over the past several weeks, the Family Healthcare Foundation participated in more than 60 outreach events across West Central Florida, connecting with families where they live, work, learn, and gather.

We shared information about affordable healthcare coverage, answered questions, connected families to resources, and helped make a complicated healthcare system a little easier to navigate.

We couldn’t have done this without our entire team 💙 They were flexible, supported one another through a very busy season, and never lost sight of why we do this work: making sure families have the information and support they need to access care.

We’re grateful to every school, organization, community partner, and event that welcomed us.

Thank you to the Hillsborough County Public Schools School Nurses for the opportunity to share our Navigating Healthcare in Hillsborough County presentation.

Our Connecting Kids to Care Program Manager, Marcela Jimenez Ramirez, led an engaging discussion on navigating health coverage options for children and families. A special thank you to Michelle Ray and Kathmanda Edwin for sharing their expertise and thoughtfully answering the many complex questions from attendees.

School nurses play a vital role in connecting students and families to the care they need, and we are grateful for the opportunity to partner with them.

A special thank you to our Board Member and Supervisor of School Health Services, Dr. Maria Russ, for her leadership and for including the Family Healthcare Foundation in this important conversation.
