Referral forms are available at Mariam’s Bazaar and The Islamic society of Tampa Bay Office
Complete the referral form, with an attached copy of your identification
Drop off form Monday through Saturday 12:30—5:30 (813) 850-6351
Care Packages may take up to one week to fulfill
We will call you when your package is available
We accept donations as we are a privately funded organization.
