The Santa Clara Chapter of the California Association of Health Facilities is a non-profit organization whose members are dedicated to serving those in a long-term care setting. CAHF was founded in the 1950's and its membership comprises of more than 1250 licensed facilities in the state of California. The Santa Clara Chapter serves providers in the Monterey, San Benito, Santa Clara and Santa Cruz counties of CAHF

CAHF is now accepting award nominations to recognize individuals who go above and beyond in their role every day. However, time is running out – the deadline is Friday, August 15!
This is a wonderful opportunity to recognize and celebrate the outstanding contributions of individuals dedicated to the long-term care sector. The nomination process is straightforward, and the recognition can make a significant impact.

· The Facility Super Star Award honors dedicated employees of CAHF member facilities with over 10 years of service in long-term care. CAHF's Board of Directors selects the top 20 nominees for the award.

· The Excellence in Programming Award acknowledges exceptional achievements in various areas of long-term care, such as activity programs, community involvement, family engagement, quality assurance, resident councils, special services, and staff development.

· The Del Callaway Award recognizes a long-term care administrator who exemplifies exceptional leadership and mentoring, actively contributing to the improvement of patient care and the enhancement of overall quality. The recipient of this award is chosen through a selection process involving past awardees.

· The Nurse of the Year Award celebrates an outstanding registered nurse or licensed vocational nurse who has made significant contributions to the field of long-term care.

· The Thomas E. Spindle Memorial Award recognizes association members who have shown exceptional commitment and provided exemplary service to CAHF at the facility, Chapter and statewide levels.

WORKFORCE STANDARDS PROGRAM

Please join us for the Santa Clara CAHF Chapter April 25th, 2025 meeting. This meeting will cover the Workforce Standards Program.

Summary:

Providers are now receiving updates to their rates for both calendar years 2024 and 2025. This session will provide an overview of the status of the Workforce Standards Program, detailing the pathways, timelines, and actions to take if your facility’s situation changes. Please bring any questions you may have.

About Jenny:



Jenny Treis is currently the Assistant Director of Reimbursement for the California Association of Health Facilities (CAHF). In this role, Jenny is responsible for assisting in the development and implementation of the organization’s policy objectives related to federal and state government-related reimbursement funding for long-term care facilities. Additionally, Jenny collaborates closely with CAHF’s managed care policy specialist in implementing the transition to Medi-Cal managed care plans through the California Advancing and Innovating Medi-Cal initiative. Jenny actively advocates for CAHF and its membership on policy positions with state policy-makers and government agencies. Jenny joined CAHF in November 2023.

Volunteer of the Year - The Volunteer of the Year awards recognize youth, adult and group volunteers who contribute their time and effort to residents of long-term care facilities.

Lifetime Membership - Life Membership in CAHF is designed to provide lasting recognition to those that have provided outstanding or exemplary service to the association through involvement at the chapter and state level, as well as providing community service.

CAHF and QCHF's yearly awards for exemplary service in Long Term Care.

CAHF is now accepting award nominations for the Summer Conference, including the Showcase Award and Volunteer of the Year Award.

SHOWCASE AWARD
This award is designed to recognize facility, company, chapter or regional efforts promoting the positive image of long-term care in four categories: through media relations, community activities, social media or grassroots events.

Criteria:
· Creativity
· Ease of replication
· Number of CAHF members involved
· How the event generated positive attention
· Examples of social media activities include (but are not limited to) regular Facebook or Web page postings. Using Facebook or a website to support a local cause and uploading a video to YouTube that highlights best practices in long-term care.

VOLUNTEER OF THE YEAR AWARD
This award is designed to recognize youth, adult and group volunteers who contribute their time and effort to residents of long-term care facilities.

Criteria:
· Volunteers at CAHF member facility
· Must have provided volunteer services or participated in a series of activities with a facility for at least one year
· Each CAHF member facility may nominate one volunteer per category (Youth, Adult, and Group)
· How the event generated positive attention

At our CDPH presentation last month, nurse evaluators mentioned F700 in one of their most cited deficiencies. There was a lot of discussion and some confusion. Please join us so we can keep our residents safe, stay in regulatory compliance and provide the quality care our residents deserve.
