Can you spare 5 minutes to support youth in our community—and enter for a chance to win a $100 Amazon Gift Card?

One Prevention Alliance (OPA) would like to hear from community members living within the Stevenson-Carson School District about youth substance use in our area. Your feedback will help guide and strengthen prevention programs that support the health and well-being of our youth.
The survey is completely anonymous and takes only about 5 minutes to complete. Responses will be used to help OPA develop program goals, plan community events, and support funding for prevention efforts.

Take the survey here:

Want to enter the drawing for a $100 gift card?

After completing the survey, take a screenshot of the final “Thank you for participating in this survey.” screen and email it to [email protected] with the subject line: SURVEY COMPLETE.
Your screenshot will only be used to confirm completion and is not connected to your survey responses.

We will SPIN THE WHEEL for this prize on Wednesday, June 17!
