🫀 What can the heart muscle outside an infarct tell us about recovery and future cardiovascular risk?

A review published in JACC: Cardiovascular Imaging explores why noninfarcted myocardium deserves closer attention following an acute myocardial infarction (AMI).

Using cardiac magnetic resonance (CMR), researchers can identify inflammation, structural changes, and altered heart muscle function in areas beyond the infarct itself.

Results from the CECCY trial provide new insight into the role of carvedilol during anthracycline therapy.

Carvedilol did not reduce the decline in left ventricular ejection fraction over 96 weeks, but it was associated with possible attenuation of diastolic dysfunction and cardiac remodeling.

These findings add important nuance to ongoing efforts to prevent cardiovascular injury in patients receiving potentially cardiotoxic cancer therapies.

A complex congenital heart disease case in JACC: Case Reports highlights the importance of careful hemodynamic assessment and staged management.

A 17-year-old patient presenting with syncope, chest pain, and a 12.5-mm patent ductus arteriosus underwent successful closure with a 16-mm VSD occluder after pulmonary vasodilator therapy improved pulmonary vascular resistance from 7 to 5.5 WU. Following intervention, symptoms resolved and left ventricular remodeling improved.

The case underscores how thoughtful patient selection and reassessment of pulmonary vascular resistance can help guide intervention in challenging PDA presentations.
