Did you know that advance care planning is about more than paperwork?

At its heart, advance care planning is about having meaningful conversations with your loved ones about your healthcare wishes should you become seriously ill or unable to communicate. Many people also choose to document those wishes through advance directives.

The conversation is often the most important step.

Health equity in hospice and palliative care is such an important conversation, and one we believe deserves more attention.

Access to compassionate, quality end-of-life care should not depend on someone’s zip code, race, background, language, or financial situation. Yet many communities still face barriers to education, support, and care.

At Hospice Foundation for Outreach, we believe advocacy, education, and honest conversations are key to creating a more equitable future for individuals and families navigating serious illness.

We encourage you to take a few minutes to explore these articles and think about what equitable end-of-life care should look like in our communities. đź’™

Sending your kiddo off to college comes with a lot of excitement and a lot of new “adult” responsibilities, too.

One important conversation to consider before move-in day? Advance healthcare directives.

Once your child turns 18, parents may not automatically be able to make medical decisions or access health information in an emergency. Completing these documents ahead of time can provide clarity, peace of mind, and help ensure their wishes are known.

It’s not always the easiest conversation, but it’s an important one. 💙

May is Mental Health Awareness Month đź’š

Navigating serious illness, caregiving, grief, and end-of-life care can take a significant emotional toll on individuals and families. Mental health is an important part of overall well-being, and compassionate support matters at every stage of life.

At Hospice Foundation for Outreach, we believe in creating space for honest conversations, emotional support, and connection during life’s most challenging moments.

You are not meant to carry it all alone.

There’s a growing conversation happening around end-of-life doulas and whether there should be more standardized training and regulations within the field.

As more individuals and families seek additional support during serious illness and end-of-life care, this debate raises important questions about education, quality of care, and how these roles fit alongside hospice and palliative care teams.

What are your thoughts on this conversation? đź’™
