One Center is located at 1000 Wilbraham Road, Springfield, with a satellite office at 3 State Street in Westfield, and a second center located at 260 Westfield Road, Holyoke. While the centers may have some variation in services due to available funding, population and other demographics, each center does provide the following: information and referral services, family trainings, family networking

“A Family Centered Home Health Agency”
MCS has a home health agency located at 96 Industry Ave, Springfield. The goal of Bridging Care for Families is to uniquely support families who are striving to care for a child with significant medical needs. We believe that family centered nursing supports are critical in complementing families’ capacities to provide care at home. In addition, our goal for skilled services is to provide the highest quality nursing and therapy services to our clients to ensure they reach their optimal level of health and well being. Information and Referral Services
These services are provided to families of children and adults with developmental and intellectual disabilities. Staff assists families in identifying the most appropriate resources and service options available. They also help to identify financial or other state services for which families may be eligible and offers guidance on how to navigate those service systems. Families are welcome to access the centers’ “library of resources” which has available a variety of resources that can assist families and individuals looking to access or learn about different services or supports. Family Trainings
Topics are offered to address issues and concerns across the family lifespan. Families provide input and guidance to determine what trainings they feel would be of interest. Unique trainings series are offered in small group formats to accommodate cultural/linguistic and geographic interests, issues and concerns. Networking and mentoring groups are encouraged and supported by MCS. These groups offer members opportunities to explore issues, problem solve, or to organize events and activities. Some topics currently being addressed by various parent networking groups include: planning for the future, transition, grandparents raising a grandchild with a disability, and culturally specific issues and concerns. Community Connections and Resources
MCS actively affiliates and partners with provider agencies and community resources such as The Boys and Girls Club, The YMCA, Best Buddies, The Jewish Community Center, and Special Olympics in order to expand opportunities for individuals with disabilities. Service Navigation
Service Navigation is the provision of comprehensive and individualized information, guidance, and support. MCS navigators help families to identify and address their needs, connect to potential resources, problem solve, and navigate the service system.
