SIU Medicine in Springfield had the pleasure of having two graduate level dietetic interns this summer from SIU-Edwardsville. One of the students, Kayla Liggon, wrote an article on the inhaled insulin option. Is anyone using this? Any feedback on the inhaled option?

Afrezza-Inhaled Insulin
Background:
Afrezza is an inhaled insulin that is used for adults with type 1 and type 2 diabetes. This is a rapid-acting insulin that has been approved by the FDA and is designed to easily manage blood glucose levels. Afrezza is taken through an oral inhaler and requires no daily injections. This rapid absorption of insulin is through the lungs, which allows for delivery into the bloodstream in less than 1 minute.
Afrezza vs. other insulin delivery systems:
After looking at other insulin dosing products, Afrezza seems to be the most discreet and rapid acting insulin on the market. Afrezza works quicker than most insulin delivery systems. After inhaling, insulin enters the bloodstream within 1 minute and will begin lowering blood glucose levels within 12 minutes. Depending on dosage, peak effects of Afrezza will occur within 35-55 minutes after dosing. Upon dosing, Afrezza will clear the body within 1.5 to 3 hours, decreasing your risk for post meal hypoglycemia.
Availability:
Like many other insulin delivery systems, you are required to have a prescription from a healthcare provider to be able to use Afrezza. First, you must consult with a healthcare provider to prove that Afrezza is a suitable product for you. You then will be required to take a breathing test, to ensure your lungs are functioning properly for the product. If passed, a healthcare provider can send in a prescription and provide a tutorial on use and dosing related to Afrezza. Many insurance companies do cover Afrezza.
Limitations:
Although Afrezza can be very convenient for some, this product is not suitable for everyone. Since this is inhaled insulin, this product is not recommended for smokers, or those with lung diseases such as COPD or asthma. To ensure that Afrezza is a suitable product, those interested are recommended to take a breathing test to check their lungs are functioning properly. There are also minor side effects reported with Afrezza. Some include hypoglycemia, cough and throat irritation, and bronchospasms. Afrezza is also such a new product, that you must work alongside a healthcare provider to ensure dosing is correct and to see how your body responds to inhaled insulin.

In the U.S., fall is the start of flu season, and influenza can be unpredictable and serious, even if you’re healthy. Prevention is the best way to protect yourself and your family, so make sure to get your flu shot and to stay home (or keep your kids home) if you suspect signs of sickness.
If you haven’t yet done so, get your COVID-19 vaccine or booster. And to make things extra easy, you can even do it at the same time as your flu shot.
There is a category of diseases called pneumococcal disease, of which pneumonia is one of the most dangerous—the other most dangerous being meningitis. People with diabetes are about three times more likely to die with flu and pneumococcal diseases, yet most don’t get a simple, safe pneumonia shot. Talk to your healthcare provider about a plan to get vaccinated.
Having elevated blood glucose increases your risk of infection so see your medical provider and certified diabetes care & education specialist if your blood sugars are above your target range for other tips to get back on track and be your healthiest self.
