What is PACE?

PACE of the Ozarks is a program designed to help older adults who qualify for nursing home level care continue living safely in their homes and communities.

We provide a full team of healthcare professionals who coordinate medical care, medications, therapy, transportation, meals, social activities, and support services. Rather than families trying to navigate multiple providers and agencies, PACE brings everything together under one coordinated care team.
