24 Hour Call/Text Line 9 Each year, nearly 30,000 Wisconsin families are confronted with a crisis - a teen runaway.

RAYS Youth Services, a program of Foundations Health & Wholeness, serves youth (ages 10-21) and their families as they navigate the challenges of the tween and teen years. We provides no-cost prevention, outreach, and intervention services including:

Mediation
Housing Referrals
Street Outreach
Youth Events
Runaway Follow Up
Youth Groups
Presentations
Referrals to community members that can help with current needs. Teens run away from home for a variety of reasons, most commonly its due to unresolved conflict caused by misunderstandings, lack of communication, and differences over roles, rules and expectations at home. Meet Heather Chillemi, our Mobile Outreach Navigator!

Heather has been with Foundations for three months and brings a unique background to her work, with a bachelor’s degree in Psychology and a master’s degree in Industrial/Organizational Psychology. After beginning her career in the corporate world, Heather spent time raising her two children before discovering her passion for human services.

Heather chose this field because she wanted a career with real meaning and purpose, both for herself and for the people she serves. She describes herself as more of a listener than a talker and is proud of the work she’s already doing with Foundations.

Outside of work, Heather loves spending time with her family, especially boating and fishing with her husband of 26 years. She also enjoys traveling, Elvis Presley, crime documentaries and reading autobiographies.

Fun fact: Heather once created a product that was sold at Target! It may have been a flop, but she still made $2,000 from it. She also appeared on a reality show that, in her words, was “also a flop.” Heather’s next big goal? More travel and definitely not a cruise. 🦈

Over the next several weeks we'd like to introduce our team members!

Hi, I'm Jackie, Director of Foundations Youth Services!
Foundations took over this program in Sheboygan in 2020 with the goal of helping youth and families navigate difficult situations. What began as a local program has grown into a 16-county initiative serving at-risk, homeless, and runaway youth across Wisconsin.

Outside of work, my favorite title is Mom. I love watching my kids grow up, gardening, caring for my cats and chickens, and filling my pantry with home-canned food from the garden. Just don't ask me to cook it... or deal with the bugs! 🐔🐱🌱

I'm incredibly proud of how far Foundations Youth Services has come and grateful for the opportunity to support youth and families every day. But I am even more grateful for the amazing people I get to work with and alongside every day. From our staff to our community partners, they are the ones making a real impact in communities across our 16-county service area. Their dedication, compassion, and commitment to youth and families are what make this work possible, and I am honored to be part of such an incredible team.

Together, we're helping create hope, stability, and brighter futures for youth and families across Wisconsin. 💚

Foundations Youth Services was excited to be part of today’s Community Resource Fair at the GBAPS Head Start Program (West) Open House! Families stopped by our table to decorate their own First Day of School Picture Frame—a fun way to capture and hold onto those special back-to-school memories. 🖍️🎒💙 We loved connecting with families and helping kick off the new school year with a little creativity and fun! 🍎✨

🎒✨ One Stop School Shop at the Kroc Center!

We had a great time connecting with kids, parents, and caregivers today! Kids got creative making keychains 🔑, while parents and caregivers could choose a positive “Take What You Need” inspiration clip to take with them. 💛

The start of a new school year can be exciting, but it can also bring stress for kids and parents. Sometimes, a few encouraging words are just what we need. 🧡
