AGEnts for Change-Santa Clara County Passionate volunteers work to support key priorities for aging with dignity, choice and independence in Santa Clara County.

Our focus areas include: Pedestrian Safety; Securing adequate funding for senior services; Improved information access; Supporting "no gaps" in moving Medicare and Medical beneficiaries into Managed Care Plans (Dual Eligible Care)
AGEnts for Change is part of The SCAN Foundation’s Community of Constituents initiative, building a statewide movement to transform the system of care so that all Californians can age with dignity, choice and independence.
