Serving as a vital link between SJSU resources and the Santa Clara County community, offering free services, education, and resources to foster health equity and holistic well-being for under-resourced children, youth, and families. Our Mission:

Located in South San Jose at Oak Grove High School, the HDCC is a unique interprofessional community clinic, promoting equity through wellness for child

ren, youth, and families in the lowest income areas of Silicon Valley. How we do it:
- Provide high-quality physical, psychological, communicative, and relational wellness through screenings, brief assessments, short-term services, education, and referrals.
- Community-based for accessibility, community need-driven; and locally networked.
- Offer culturally-sustaining and trauma-informed practices, building on existing strengths, protective factors, social supports, and cultural resources.
- Create interprofessional internships and practicums for SJSU students.
- Bridge physical, cognitive, and socio-emotional domains.
- Research-to-practice implementation for our students and faculty.

Last month, SJSU HDCC partnered with SJSU Human Rights Institute to host Growing Together, a free community event featuring food, Ballet Folklórico, resources, and fun activities for families. 🌱💃🏽✨

Summer is almost here, and we’re ready for it 😎☀️

Neighbors in Nature | Summer Edition starts on June 9th! Join us every Tuesday and Thursday for FREE games, sports, arts activities, snacks, and lots of summer fun!

Come connect with family, friends, and neighbors; enjoy the outdoors; and make new memories with us this summer 🌿

NEIGHBORS IN NATURE - for youth and families
+ Tuesdays & Thursdays for 8 weeks (June 9th-July 30th)

Additionally, Poder de Familias (parent and caregiver workshops) will be starting up again on June 25th! These are optional discussions you can participate in while youth play at Neighbors in Nature. Workshops will cover various family-strengthening skills 💞

Adolescent self-esteem - when parenting teens with low self-esteem, it’s important to create a safe and supportive space by listening without judging and reminding them of their strengths.
